Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976.  Our spectrum of services has grown from the delivery of meals-on-wheels to a full range of community support services, covering the whole Greater Toronto Area, York Region and Peel Region.


We are currently seeking for a Nurse Practitioner to join our team.



  • Plan, coordinate and oversee GAIN (Geriatric Assessment & Intervention Network) community hub services for clients with complex needs
  • Provide specialized, comprehensive inter-professional geriatric assessment and work with seniors and their families to develop a personalized care plan to assist the seniors to remain in their homes
  • Collaborate with primary care providers and other services in implementing care plans
  • Is responsible for autonomously diagnosing, ordering and interpreting diagnostic test(s), prescribing pharmaceuticals, and performing specific procedures within the legislated scope of practice
  • Provide direct patient care focussing on health promotion and maximizing clients' safety and function to support frail seniors living at home
  • Actively participate in team meetings, liaise with community based providers, advocate on behalf of clients and their families, and participate in inter-professional conferences
  • Identify and document occurrences and make corrective actions when required


Job Requirements

  • Completion of recognized primary care or nurse practitioner program
  • Master of nursing; current extended class certification with College of Nurses of Ontario
  • Sound working knowledge of nursing function, health care and geriatric care
  • 3-5 years experience in clinical work with geriatric clients
  • Demonstrated ability to exercise independent judgement and to assess problems efficiently and effectively
  • Ability to work well within an inter-professional team and to foster effective working relationships
  • Ability to exercise initiative and observe strict confidentiality
  • Proficiency in English; a second language such as Chinese reflecting local community is an asset
  • Valid Ontario driver's licence and own vehicle for home visits
  • Flexible work hours including evenings and weekends
