Position / Poste: Senior Acquired Brain Injury Rehabilitation Worker-14 Month Contract-York Simcoe Brain Injury Services (Full Time) 

 

Duties / Fonctions:

  • In consultation with the supervisor, develop and implement support plans/IRP’s for consumers with ABI
  • Assist consumers to gain greater autonomy in activities of daily living such as, personal hygiene cognitive and problem solving skills, support with communication skills, community orientation/ integration while supporting and enhancing the overall improvement of quality of life
  • Reporting responsibilities, i.e. progress notes, Incident reports, data collection and filing
  • Ongoing assessment of client’s relevant skill strengths and needs
  • Develop, coordinate and implement the delivery of specific services within the community
  • Liaise with other social service agencies and health care providers involved with consumers
  • Participate and provide training as required• Will be required to manage and schedule caseloads

 

 

Qualifications / Compétences requises:

  • Post-Secondary Education in Social Services or equivalent experience
  • Current First Aid/CPR Certificate
  • Previous experience working with ABI consumers is an asset
  • Ability to speak, read and write English
  • Ability to assess consumer needs, translate this into a service plan & establish/maintain appropriate controls
  • ASIST training is an asset
  • Experience with Microsoft Office
  • Good interpersonal, organizational and communication skills, including de-escalation techniques
  • Professionalism in dealing with other service providers
  • Demonstrated team building skills
  • Valid Driver’s License and access to reliable vehicle
