Your role would involve:

  • Supervise, direct and provide ongoing coaching, mentoring and recognition to the Transportation, Meals on Wheels (MOW), Community/Client Services, and the Home At Last (HAL) Programs.
  • Manage the Community & Client Services Department on a day to day basis, monitor targets and units within each assigned department and implements strategies to correct or manage as needed. Ensure quality service delivery and customer service excellence across the programs.
  • Oversee the collection, monitoring, and reporting of data and quality performance indicators weekly, monthly and quarterly as required. Develop, monitor and evaluate strategies to meet funder targets.
  • Assist with annual budget/operational planning.
  • Lead for the implementation of initiatives related to agency improvements linked to Accreditation Canada QMentum Standards. Support best practices as well as the delivery of safe, effective, efficient and quality client care services.
  • Represent the agency at external/internal committees, planning tables, and at various networking initiatives.
  • Provide after hours on-call support as required. 

The successful candidate will:

  • Have a Master in Social Work and is a member in good standing with the College of Social Work. Education in Nursing is an asset.
  • Have a minimum of five to seven years of experience within the community sector or health care environment.
  • Have a minimum of 5 years supervisory/leadership experience in the community sector strongly preferred. Must possess strong leadership, motivation, delegation, coaching/mentoring and empowerment skills.
  • Have experience working with the frail and elderly population and knowledge of community resources including various funding sources.
  • Demonstrate strong program management and effective problem solving skills, ability to deal effectively with complex and ambiguous situations.
  • Have proven ability to establish and maintain productive working relationships internally and externally with a wide range of stakeholders and partnering organizations.
  • Have excellent communication (written and oral), team building, organizational, conceptual and interpersonal skills.
  • Possess critical thinking skills, sound decision making and a collaborative approach. 
  • Have excellent computer skills with proficiency in MS Word, Excel, Outlook and Power Point.
  • Have knowledge of CIMS and the RAI-CHA is an asset.
  • Valid Ontario Driver’s License and access to a vehicle is required.
    Current Vulnerable Sector Police check (completed within last 12 months) and a current First Aid / CPR Certificate is required.
