Position / Poste: Rehabilitation Services Coordinator-Full Time

 

Duties / Fonctions:

The RSC is responsible for the delivery of coordinated in home and community support services to individuals with acquired brain injuries and their families/support network. The RSC will lead a team of Acquired Brain Injury Rehabilitation workers and will focus on individualized needs for those we support.

  • Reporting responsibilities (incident reports, behavioral data collection and verbal reports)
  • Participate in any required training
  • Ensure compliance with ABI services , policies and procedures
  • Communicate with consumer, family, and team members
  • Work closely with the behavior consultant to ensure that strategies given are implemented with consistency amongst the team
  • To provide services on a flexible basis including evenings and weekends, providing back up support when staff are absent
  • Responsible for leading weekly team meetings
  • Case management required
  • Develop and implement staffing plan
  • Manage and approve staff hours in accordance with pre-approved budget and program needs
  • Support the supervisor in recruitment of new hires
  • Responsible for training all new hires
  • Oversee and support the team with Individual Rehabilitation Plans
  • Support with Carrying an afterhours phone
  • Complete and ensure annual service agreements are up to date and reflect accurate service as per the consumer’s needs
  • Complete all probationary and annual performance reviews

Qualifications / Compétences requises:

  • Post Secondary diploma in Human and Social Services Program with minimum of two years’ experience in Human Services field and/or working with people with disabilities
  • Ability to speak, read, and write in English
  • Current CARE and CPR, First Aid an asset
  • Proven consistency in work performance, including reliability and flexibility
  • Excellent interpersonal communication and organizational skills
  • Sound behavioural management skills, cognitive problem solving skills.
  • Working knowledge of Acquired Brain Injury is an assest
  • Experience in report writing an asset

Salary Range / Échelle des salaires: Based on experience and knowledge
