Purpose

The Family Liaison works with families, individuals, service providers and other stakeholders to identify appropriate resources and support connecting people in need with those resources. The Liaison empowers people to build a meaningful life and provides the necessary supports to allow individual and families to reach shared goals while focusing on the development of the whole individual. The result of these efforts is to decrease caregiver stress, build capacity in families and individuals, and ensure all family needs are met.

Key Functions

  • Connect new and returning families to additional services within AdaptAbilities.
  • Advocate for additional funding and services, and refer families to AdaptAbilities community partners when appropriate.
  • Be a resource to families, participants and AdaptAbilities’ Program Coordinators.
  • Conduct initial intake meetings with individuals and parents to determine needs required in each case.
  • Understand AdaptAbilities’ policies and processes and communicate them effectively to both staff and families.
  • Identify and solve problems with demonstrated problem-solving skills.
  • Develop and foster genuine relationships and be passionate about participants’ lives.
  • Conduct general administration, follow-ups, documentation and reporting duties.

Qualifications

  • Three years hands on direct experience and a minimum of one year supervisory experience. 
  • Completion of a diploma/degree in the human services field.
  • Experience with individuals with complex behaviors.
  • Excellent written and verbal communication skills.
  • Strong organizational skills and problem solving abilities.
  • Flexible, comfortable working independently and leading small teams.
  • Clear Police Information Check and Intervention Record Check. 
  • First Aid Certification with CPR, Non-Violent Crisis Intervention, Abuse Prevention and Response Protocol.

 

Working Conditions

This is a fulltime permanent position with a total of 40 hours per week. Flexible days and office hours are Monday- Friday 8:00- 4:30 pm.

 

Compensation Package

$19.10 to $24.60 per hour.
