The Fusion Success Coach is responsible for providing individual and family coaching, assessment, and advocacy to youth aged 15-24 who have experienced episodic or chronic homelessness and disconnected from family and natural supports. This position works collaboratively with other agencies and community groups to identify ways youth can explore activate and/or strengthen connections to natural supports. The position will assist the youth to increase their social capital and locate appropriate community referrals to promote a positive and successful transition to adult self sufficiency. Responsibilities include but are not limited to:


  • Assists youth and their natural supports in developing problem solving, conflict resolution and communication skills
  • Advocates for youth and their natural supports to promote increased functioning and successful transition to adulthood
  • Works collaboratively with other agency partners to provide case management to youth and supports
  • Facilitates service planning meetings, case conferences, and parent trainings sessions as necessary
  • Assists youth through relationship based case management, to identify their strengths, needs and goals
  • Support youth and supports in the development and implementation of action plans to address goals
  • Provide information, articles, books, and other materials needed to assist natural supports in learning more about parenting, adolescent development, and other applicable topics
  • Document, implement and revise service plans, progress and discharge reports in cooperation with the family and other involved programs
  • Acts as a liaison between families, social workers, school representatives, doctors and other professionals involved with the youth and their supports to promote the self-sufficiency
  • Maintains and develops professional relationships with external agencies/groups such as community associations, schools, law enforcement and social service agencies


  • Degree or Diploma in Human Services or a related field, with 2-3 years experience
  • Must have experience working specifically with families experiencing challenges with adolescents
  • Experience working with high-risk youth
  • An understanding of the homeless sector in Calgary, Calgary’s 10 Year Plan to End Homelessness and Plan to End Youth Homelessness in Calgary
  • Possess knowledge of Child and Family Services and the Family Enhancement Act
  • Proficient computer knowledge of Microsoft Office as well as excellent oral and written skills
  • Able to work a flexible schedule that meets the needs of the families
  • Current Standard First Aid and CPR
  • Criminal Check with Vulnerable Sector Search and Child Intervention Record Check
  • Valid Class 5 Drivers License with an acceptable Drivers Abstract
  • Use of a reliable vehicle with proof of $1,000,000 liability coverage

Please Note: Successful candidates will complete criminal record check and child intervention record check at the time of new hire paperwork.  A criminal record check and child intervention record check dated within 6 months of the interview date will also be accepted.
