Who we are:        Youth Empowerment & Support Services (YESS) is a not-for-profit Agency working with youth facing difficult realities.


We are currently recruiting for the position of Case Coordinator.   The ideal candidate will be friendly, have strong interpersonal, written and verbal communication skills, a high level of attention to detail and strong relationship building skills that reflect calm assertiveness, and consistency.  This position has alternating weeks of days and evenings schedule.


The job requires designing and implementing of day-to-day support and case management for a maximum of eight youth who are in YESS programs and a varying after care caseload for youth who transition out of YESS.


Reporting to the Residential programs supervisor, your main responsibilities will be:

  • Developing goals with the youth
  • Acuity Assessment review and follow up
  • Identifying and securing natural and professional supports for each youth
  • Working through and monitoring process towards goals
  • Monitoring and modifying supports as required
  • Safety, crisis, and transition planning with youth and their support networks
  • Ongoing regular meetings and communication with youth and their supports



  • Certification, Diploma or degree in Child and Youth Care or other related Human Services with at least two years related human services experience; preference for those working with youth facing difficult realities
  • A certification in High Fidelity Wrap Around is considered an asset for this position, however, position specific training will be provided by YESS


Other requirements:

  • Criminal Record check and child intervention record check (obtained less than six months ago)
  • Valid driver’s license.
  • Knowledgeable and respectful of values, beliefs and practices of Aboriginal and other cultural groupsF
  • Basic computer skills, First Aid Training, Suicide intervention, Non-Violent Crisis Intervention, ETO, will be an asset
  • Ability to monitor and prioritize tasks, be a team player, tactful and diplomatic


Salary:   Based on education and experience.   Agency paid health benefits after six months of employment
