Case Management Coordinator

Our client, Main Street Project is committed to serving the needs of Winnipeg’s most vulnerable residents.  Feeding all kinds of needs from body and mind to shelter and long-term care, Main Street Project wouldn't be able to build stability within their community without the efforts of their team and so many other dedicated people.  They are now seeking another dedicated staff member to join their team as a Case Management Coordinator.

 

The Case Management Coordinator will oversee the case management team and establish case management standards and implement work across the agency.

 

THE ROLE:

 

  • Work collaboratively with internal and external stakeholders.
  • Provide coordination and guidance to 12 direct reports and staff across the organization as appropriate.
  • Implement Case Management policies and procedures, standards and practice throughout the agency, programs and case management team.
  • Support the clients’ achievement of safe, realistic, and reasonable goals within a complex health, social and fiscal environment.
  • Interact with the client actively to engage the community member as the primary source of information.
  • Case planning.
  • Team work – work within a multi-disciplinary team within Main Street Project.

 

YOU AND YOUR EXPERIENCE:

  • You ideally have completed a Masters in Social Work or in social services related field.
  • You have a minimum of 3 years’ experience managing a team in the case management or social services environment.
  • You maintain a current professional social work licensure /certification or national social work certification.
  • You have knowledge of mental health diagnosis, and co-occurring disorders and have the ability to manage, assess and work with challenging behaviors as well as provide guidance and support to a team of case workers.
  • You are a strong leader and have experience with HR practices such as coaching and performance management.
  • You have experience with training and development in the social services field.
  • Your organizational skills are superior and you are able to manage multiple projects at the same time.
  • You have a client focused approach, a passion for community and are service oriented.
  • You have experience in Homeless Individuals and Families Information System (HIFIS) or similar healthcare reporting systems.
