Assertive Community Treatment Team – Casual Position (Licensed Practical Nurse or Support Worker)

 

Boyle McCauley Health Centre’s Pathways to Housing Program in Edmonton ends homelessness for people who suffer from serious, persistent mental illness (such as schizophrenia and bi-polar disorder), usually co-occurring with addictions and medical and social problems, by:

 

  • providing access to permanent independent housing without requiring treatment or sobriety as a precondition for housing; and
  • providing outreach community support and treatment with a philosophy that is founded in the principles of consumer choice and harm reduction, using both evidence-based Best Practices and innovative approaches.

 

Our inter-disciplinary Assertive Community Treatment (ACT) team:

  • provides comprehensive wrap-around treatment, support and rehabilitation/recovery services;
  • uses a team case management model to form treatment/recovery plans as a team using a person-centred approach;
  • provides assistance with housing, life skills and tasks of daily living.

 

In a Casual position, staff are focused primarily on generalized supports.  Although the range of work with Clients can be quite broad, common examples would include taking Clients grocery shopping, transporting and accompanying Clients at medical or other appointments, assisting Clients to set up and maintain their homes, assisting Clients to engage in the community, and taking steps to support Clients’ tenancy. 

 

LPNs in a Casual position will also play a role in medication administration, assessment of medical issues, and supporting the medical team, including the Psychiatrist and Family Physician, in meeting the medical needs of Clients.

 

Please note that the work is community-based, so most Client interactions take place in Client’s homes or elsewhere in the Community.  Staff must be comfortable working independently, and with driving throughout Edmonton.

 

This is a Casual position.  Shifts will be scheduled between the hours of 8:30am-4:30pm, Monday thru Friday.  Initially, we are looking for candidates available for Wednesdays and/or Fridays, as well as other hours as needed.

 

Salary range is  

LPN - $21.76-$27.78 per hour

Support Worker - $18.45 to $23.55 per hour

 

 

Qualification Requirements

 

  • Registration with CLPNA (LPNs) or a 2-year diploma in a field related to social services, mental health and/or addictions. A relevant combination of education and experience may be considered.

 

  • Experience in one of more of following areas would be an asset:
    • acute psychiatry and crisis intervention/emergency services;
    • mental health services in a community treatment setting;
    • Assertive Community Treatment;
    • working in addictions, concurrent disorders and/or Housing First; and/or
    • providing outreach services in a ‘street/inner city/chronic homelessness’ environment or equivalent.

 

  • Knowledge of best practices in
    • working with people with complex co-occurring serious mental illness, addiction and medical issues;
    • client centered, strength-based recovery treatments/rehabilitation strategies;
    • Housing First/Pathway to Housing models;
    • Harm Reduction philosophy;
    • working from a culturally appropriate, trauma informed and client-centred perspective, including how best to support Indigenous Clients.

 

 

  • Successful experience in inter-disciplinary teamwork.

 

  • Communication skills/literacy: written, oral, computer

 

  • Valid driver’s license and use of personal car is a must (Kilometrage will be reimbursed)
