Registered Practical Nurse, Primary Health Care

Contract Position: 35 hours per week

Contract Period: April 2018 to May 2019

Salary Range: $41,971.00 per year to $50,051.00

Job Summary

 

The Registered Practical Nurse works under supervision of the Director, Clinical Programs and performs assigned duties within the objectives of the program and the standards of the CNO. Also provides clinical care, performs range of medical and laboratory procedures as approved for delegation, maintain equipment and medical, laboratory and pharmaceutical supplies.

 

The incumbent will ensure that all organizational policies are followed.

 

Primary Responsibilities

  • Perform medical and diagnostic procedures such as venipuncture, electrocardiogram, spirometry, immunization, allergy injections, urine BHCG, ears irrigation and throat swabs on as needed basis working as a part of a busy team of MDs and NPs. Integrate obtained information using analytical thinking.
  • As delegated by providers, conduct health assessment, including history & partial well adult/child exam. For example: vital signs, Snellen eye exam, BMI. Ensure that task work performed correctly, thoroughly & in timely manner.
  • Arrange for referrals, including emergency referrals and follow up appointments for medical specialists or ER. Initiate and maintain external communication, collect required medical and diagnostic documentation and communicate according to privacy code.
  • Maintain accurate and timely records and encounter data in patients EMR. Participate in chart and case review. Educate clients and provide information about available community resources.
  • Triage urgent care phone calls, collect and communicate information to assigned provider and perform timely follow up with the caller. Using analytical thinking, take personal responsibility to match clients’ needs to available services.
  • Do the home visits for older clients and clients with disability for checking vital signs, injections or education on as needed basis.
  • Dispose biohazards wastes, including regular check and replacement of biohazard waste equipment in PHC exam rooms and offices, maintain efficient communication with Stericycle regarding biohazard waste pick up and MSDS and safety literature supply.
  • Always preserve confidentiality of clients’ private information.
  • Receive and manage internal and external communication regarding intake process.
  • Identify problematic intake cases, elaborate decision based on Primary Health Care team discussion.
  • Book new clients for providers taking under consideration the provider’s availability & complexity of the client’s needs.
  • Provide orientation for new clients, insure that the clients understand and appreciate such a valuable intake documents as Service Agreement and Payment Agreement and build rapport with the clients.
  • Offer guidance and support to find external resources for ineligible clients.
  • Stock and maintain medical and laboratory supplies in exam rooms, including ordering pharmaceutical samples and making orders from Medical Mart and Public Health on an as needed basis.
  • Order and maintain up to date supply of current medical literature for educational, preventative purposes. Take active role to educate clients and promote clients self-care.
  • Actively participate in supervisors meetings, build open and cooperative relationship and collaboration across programs.
  • Work towards to maintaining harassment free work environment.
  • Build and maintain strong business relationships with partner organizations, participate in outreach to promote LAMP’s philosophy and seek for future opportunities of partnership.
  • Seek for a feedback from peers and management and use it for professional and personal growth.

Required Qualifications

  • Registered Practical Nurse Diploma
  • Registered and in good standing as Registered Practical Nurse with the College of Nurses of Ontario.
  • Three to five years nursing experience working in a community or public health primary care preferably setting.
  • Experience in supervising staff in a relevant setting.
  • Thorough knowledge and understanding of medical/laboratory and charting procedures.
  • Excellent interpersonal skills necessary to work effectively across all levels of the organization’s diverse workforce.
  • Excellent communication skills, both verbal and written, with the ability to clearly convey information and ideas.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team oriented atmosphere.
  • Demonstration of knowledge, understanding and practicing of current infection control and prevention guidance.
  • Recognizing LAMP’s culture, values and philosophy when dealing with clients with different backgrounds and perspectives.
  • Open minded, eager and willing to constantly learn and improve oneself.
  • Current Driver License is an asset. 
  • Knowledge of LAMP Community Health Centre, the South Etobicoke community, and/or a second language that is reflective of the community being served are all preferable assets.

Proficiency in the Following Computer Skills

  • Solid working knowledge and ability to navigate a PC windows environment, including shared drives.
  • Strong skills and experience using Microsoft applications (Word, Outlook, Excel, PowerPoint & Publisher).
  • Strong skills and ability to easily navigate the internet/intranet environment.
  • Experience and accuracy using database applications (i.e. Electronic Health Records – Purkinje & Nightingale, funder databases, etc.)
  • Ability to learn new software quickly and willingness to continuously develop technology skills.

  • Flexible working hours from 9 am to 8 pm during week days.
  • Travelling to multiple locations on regular basis for attending meetings, trainings, workshops, outreach and home visits.
  • Ability to effectively understand the appreciation of guiding fundamentals at LAMP, i.e. Health Promotion, Anti-Oppression; Harm Reduction and the principles of community-based practice.

Physical Requirements

  • Frequent data entry, with extended periods of time sitting stationary in front of a monitor while keyboarding.
  • Ability to balance all job requirements in a busy atmosphere, including responding promptly to all communication.
  • Frequent standing for extended periods of time and some occasional lifting may be required.
  • Demonstrates good and safe work habits, and maintains a clean working environment.
  • Ensures that staff and clients are compliant with LAMP’s Health & Safety regulations and LAMP’s policies and procedures, including the immediate reporting of any breaches of Health & Safety or Environmental incidents, accidents or concerns to their respective manager/supervisor and/or the Director, Human Resources and Operations.
