Job Profile:

 

The Community Transition Team is a rapid response model of care to successfully transition people who have mental health and/or addiction problems and are living in vulnerable housing, to a safe, permanent living environment.   This Program provides supports to clients for a flexible period of time up to six months.

 

The Community Mental Health Care Coordinator works with clients 16 years of age and older, experiencing significant difficulties in day-to-day functioning as a result of their mental health and/or addiction problems. Client referrals will primarily come from public hospital emergency departments and in-patient units, emergency shelters, or mental health crisis bed service providers.

 

The Community Mental Health Care Coordinator will support clients to explore options, build skills and make decisions that promote recovery and self-responsibility. S/he will also establish effective working relationships with hospitals, physicians, emergency shelters, community mental health and addiction providers, housing providers, and others as appropriate.

 

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.

 

Requirements:

  • College diploma with a minimum of three years experience in a mental health environment.
  • Experience working with individuals experiencing serious mental illness, concurrent disorders and dual diagnosis (developmental disability and/or traumatic brain injury) is preferred.
  • A second language (Cantonese, Mandarin, Russian, Italian, Persian, Punjabi, Korean, Tamil, Urdu) reflecting the local community is preferred.
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements
  • Ability to apply Recovery principles and empowerment oriented philosophies and practices in work with clients
  • Knowledge of systemic issues such as poverty, unemployment, stigma and the isolation felt by individuals with serious mental illness and their families.
  • Extensive knowledge of supports and services in York Region, including formal and informal resources.
  • Demonstrated ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with clients, maintain confidentiality, and engage in reasonable self-care strategies designed to reduce stress by balancing work/life responsibilities.
  • Excellent oral and written communication skills
  • Ability to demonstrate computer skills, specifically Microsoft Office and Outlook
  • A valid Canadian driver’s license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
  • A satisfactory and current Vulnerable Sector Screening

This position reports to and is supervised by the Program Manager.
