Location / Lieu: Oak Ridges (Richmond Hill), Ontario (with regular travel throughout York Durham Region and to Toronto)

Position / Poste: Coordinator, Caregiver Support ACDP(14hrs/per week)

Duties / Fonctions:

  • The Coordinator works as a member of an inter-disciplinary team and supports the members and their caregivers of the Aphasia and Communication Disabilities Program. The team consists of Clinical Coordinator, SLP’s, CDS’s, PSWs. Provides assessment/supports, planning, consulting and collaborating with appropriate multi-disciplinary team members in the community.
  • Provides community based support to individuals living with the effects of a physical disability/Aphasia/ABI, their families and/or their caregivers within the York Durham Region
  • Supports individuals in promoting self-direction and assists in facilitating opportunities for meaningful community living.
  • Participates in partnerships and develops groups support to foster ongoing dialogue with caregivers and their support network.
  • Plans and presents caregiver training and development sessions on a regular basis, to enhance understanding and coping abilities related to specific problems.
  • Coordinator attends and participates in departmental meetings and internal/external case management conferences by liaising and conferring with other agencies.


  • Designation in Social Work (MSW, BSW), Social Service Work (College Program) or Disability Management
  • 2-5 years work experience in community mental health, with demonstrated experience working with individuals with physical disabilities and their families
  • Experience with individual/family/marital/group therapies
  • Highly developed written and oral communication skills
  • Highly developed interpersonal skills, including mediation, negotiation, advocacy, consultation and collaboration
  • Organization and time management skills
  • Knowledge of social work practice in community-based health settings
  • Good understanding of a variety of disabilities and the needs of people with disabilities
  • Ability to access, liaise with and coordinate a variety of service providers including March of Dimes Canada team, other health care facilities, community agencies and government departments
  • Proven instruction, teaching or coaching ability
  • Reliable, mature,
  • Ability to work independently
  • Knowledge of attendant services, assistive devices, accessibility standards, funding sources, community services, assessment tools
  • Access to reliable transportation
