Job Profile:

 

The Personal Support Worker (PSW) works with clients 16 years of age and older, who are experiencing significant difficulties in day-to-day functioning as a result of their mental health and/or addiction problems.

 

The PSW works collaboratively with clients, staff from emergency shelters, and other community agencies toward the achievement of identified goals and the enhancement of quality of life as defined by the client. The PSW assists the client with daily personal care needs and supports the client’s continued independent living.

 

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships. 

 

Requirements:

  •  Successful completion of the PSW certificate program
  • Four (4) years or more PSW experience working in a community environment is an asset preference is given to those with mental health experience.
  • Lived experience of homelessness is an asset
  • Strong knowledge of empowerment-oriented case management philosophies and practices as demonstrated by two or more years’ experience in this area.
  • A Second language reflecting the local community is preferred (Cantonese, Mandarin, Tamil, Italian, Farsi, Russian, Urdu, French)
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, and the Health Care Consent Act and PHIPA requirements.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.
  • Extensive knowledge of supports and services in York Region, including formal and informal resources.
  • Demonstrated ability to work collaboratively with clients.
  • Ability to use knowledge of functional abilities and mental status to informally assess these in clients.
  • Ability to demonstrate diplomacy and professionalism when working with families and other professionals.
  • Ability to manage time effectively, establish priorities, efficiently organize work, and meet deadlines by engaging in effective problem solving and decision making.
  • Demonstrated ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with clients, maintain confidentiality, and engage in reasonable self-care strategies designed to reduce stress by balancing work/life responsibilities.
  • Employs creative thinking in addressing service delivery issues.
  • Excellent oral and written communication skills.
  • Able to demonstrate computer skills, specifically Microsoft Office and Outlook.
  • Must possess a valid Canadian driver’s license and have minimum $1,000,000.00 (one million) third party liability insurance and proof of insurance coverage of personal vehicle and ability to transport clients.
  • A satisfactory and current Vulnerable Sector Screening.

  • This position reports to and is supervised by the Program Manager.
