Safehouse Transition Worker – Term Position Competition #68-18

 

ABOUT DIRECTIONS YOUTH SERVICES

 

Directions Youth Services provides 24/7 support to at-risk, homeless, and street-involved youth under 25. Vancouver’s most vulnerable youth can connect with supports, get counselling for trauma, mental health or substance issues, and transition to more permanent housing. Directions runs several facilities and programs for youth throughout Vancouver.

Directions is a division of Family Services of Greater Vancouver (FSGV), which for 90 years has provided trauma counselling and essential resources for children, families and individuals facing all kinds of life’s challenges.

 

JOB SUMMARY

 

This is a 35 hrs/week term position that reports to the Manager of Supported Housing and Transition programs. The Safehouse Transitions program operates Monday- Friday with some evening work as needed. This position provides 1-1 support to young people, 18-20 years old, during their transition to independent living.   The Transition Worker, in collaborating with the young person, identifies individual goals and the means to achieve those goals. The Transitions Worker assumes a proactive role in ensuring that all the elements of a youth-centered plan are coordinated so the youth can achieve key transitional milestones such as housing, employment, education, community engagement and life skills. 


The Youth Transitions program is based out of our undisclosed Youth Safehouse location. Directions Youth Safehouse is a voluntary co-ed residential program for youth, aged 16 – 18 years, who are in need of a safe place to stay.  This is a term position, ending March 31, 2019.

 

KEY RESPONSIBILITIES

• Create rapport and trusting, non-judgmental relationships with youth accessing the Safehouse program and those youth referred to the Transitions program.
• Create and monitor individual transitions plan in collaboration with the youth, connected resources and colleagues to address their immediate and long-term needs.
• Maintain professional, positive relationships with community and agency partners including MCFD, MSDSI, CLBC and other relevant adult support services.
• Maintain accurate logs, record keeping, and data entry related to client files and statistics.
• Responsible for maintaining knowledge of current best practices and available resources related to working with youth transitioning to independent living. 
• Ensure clear and concise communication and information transfer to colleagues and management.
• Be an active member of the Safehouse team by performing related duties. Attend and actively participate in staff meetings, supervision sessions and performance evaluation processes.
• Provide information and expertise on services and opportunities available to the youth and actively promote client involvement in the community. 
• Work with  youth in the community to explore issues relevant to developing and maintaining independent living skills
• Assist youth in accessing community supports by advocating, escorting and transporting clients to outside service referral locations as appropriate.
• Adhere to all program and agency policies and procedures. 

 

REQUIRED QUALIFICATIONS

 

Education and training:
• Completion of a Bachelor degree in Social Work or the Social Sciences
• Current First Aid and Non-Violent Crisis Intervention certificates

 

Experience:
• Minimum of 3 years experience with at-risk youth working in areas such as child protection, life- skill development, community support and/or community outreach.

 

Knowledge/Abilities:
• Knowledge of the systemic issues that impact young people transitioning out of care.
• Knowledge of child and adult social welfare services, including education, housing, social and health resources
• Knowledge of issues/needs of high risk and/or street- involved youth, including harm reduction and low barrier resources.
• Professional verbal, written and interpersonal skills.
• Valid BC Drivers licence – requires submission of abstract
• Ability to maintain a flexible schedule that allows for daytime and evening work.
