Intensive Case Management Team (ICMT)


Job Title:                   Registered Nurse

Department:              ICMT

Reports to:                Program Manager

Start Date:                 ASAP 

Salary:                       Grid 19P starting at $38.45/hr  depending on qualifications and experience.

Hours:                        Full time – 37.5 hours per week – Contract until March 31, 2019

                                    4 days on 4 days off rotating and varying 10 hour shifts including weekends and holidays

Job Summary:

Reporting to the Program Manager, the ICMT Nurse assesses, identifies, plans, implements and evaluates the nursing care required to assist ICM clients in meeting their physical, social, spiritual and psychological needs.   With an emphasis on connecting with clients where they’re at physically – on the streets, in shelters, in their homes – the ICMT Nurse provides client-centered and strengths based care in accordance with the CRNBC/CRPNBC Standards of Practice.  The Nurse performs duties such as intake, mental and physical health and addictions assessment, treatment planning, crisis intervention, consultation and counseling.



Key Duties and Responsibilities:

Assesses, plans, implements and evaluates mental and physical health nursing care for ICM clients using a framework for nursing practice

Complete in depth assessments on an outreach basis to determine stages of change, risk indicators, mental and physical health needs

Refers and connects clients for further and/or alternate specialized services, such as detox, residential treatment, community support groups (eg. AA, SMART Recovery), Opioid Agonist Therapy (OAT) clinic, outpatient Substance Use Services, Psychiatric or Addiction Physician consultation and primary care providers such as GP/NP support.  Advocates on behalf of the client to ensure respect and dignity are maintained.

Provides client-centered services by developing strength based, integrated and comprehensive care plans, helping clients formulate goals by addressing the strengths and needs of each individual.

Pursues a housing-first approach by working with the ICMT Housing Procurement specialist to secure housing placement, and rent subsidies through BC Housing.

Upon completion of appropriate training, may distribute and administer Take Home Naloxone (THN) to address those individuals at risk for overdose.

Counsel and educate clients in harm reduction concepts and practices

Maintains client records including client assessments, treatment plans and progress notes.  Ensures that access to and disclosure of information and records are in accordance with the Freedom of Information and Privacy Act.


Qualifications, Experiences and Abilities:

Graduated from an approved school of Nursing and current practicing registration with the College of Registered nurses of British Columbia(CRNBC) or the College of Registered Psychiatric Nurses of British Columbia (CRPNBC)

Minimum 2 years recent related experience, working with clients dealing with substance use, mental health  and medical concerns.

Valid Class V BC Drivers License

-   Current CPR and First Aid

Demonstrated knowledge of up-to-date diagnosis, treatment and expertise in the areas of substance use and mental health

Broad knowledge of CRNBC or CRPNBC standards for nursing/psychiatric nursing practice

Knowledge of Trauma Informed Practice, Motivational Interviewing and harm reduction

Demonstrated knowledge of the FHA Mental Health & Substance Use continuum of care

Ability to work with individuals from diverse backgrounds and the ability to be flexible and creative in providing treatment services

Ability to communicate effectively with individuals, groups and other professional within an interdisciplinary environment in a diverse community

Strong interpersonal and cross cultural communication skills

Ability to work both independently and collaboratively as a member of a multidisciplinary team

Proficient with technology, including computers, tablets, smartphones, word-processing and database management software

Physical ability to carry out the duties of the position

-  This position requires Union Membership

-  This position is open to male or female applicants.

-  This position may require working outside office hours and office environment, depending on client availability or locations.

-  Use of personal vehicle with appropriate business class insurance will be required.

-  Mileage is paid where driving for work performed is required.  Mileage is not paid for to and from work site for start and end of shift purposes.

-  Criminal Record Check is required.
