Key Overall Objective of the position

The Housing and Outreach Support Worker provides community outreach-based support services to individuals who are homeless or at risk of homelessness and to those who are experiencing a crisis related to housing, mental health, substance use or struggling with daily living due to poverty or other personal situation.  Outreach Services will include case assessment, navigation of community support services and referrals, organizing and coordinating of housing searches and placement including tenancy education and support, coaching and life skills training and case management. Outreach Workers work within a harm reduction and person centred model. All duties are performed with a multidisciplinary and multiagency team approach within the Housing First culture.

Key Accountabilities

  1. Being available to go out into the community on a regular basis to establish a rapport and meet with the clients
  2. Locating people who are in need of support services and who otherwise would not seek assistance
  3. Engaging the unsheltered homeless to establish rapport which will assist them in locating rental units or attending appointments within the community
  4. Maintaining excellent relations with health and social services professionals such as workers with the Ministry of Social Development and Social Innovation, Canada Revenue Agency, PHS Mental Health, Burnaby Mental Health Services, BC Housing, Burnaby Substance Use Services, Burnaby RCMP and other non­profit and community agencies involved in the provision of services
  5. Is an active participant and member of the Integrated case management team, and broader multi-disciplinary Housing and Outreach Program.
  6. Facilitating a full assessment of client's housing and support needs, using mandated assessment tools and templates.
  7. Developing a client centered plan with each client for the planning and pre-placement of the individual's tenancy and support requirements within the community
  8. Monitoring the areas of daily living, financial, health, social supports and resource acquisition 
  9. Working in a team environment on developing housing initiatives and finding creative solutions to housing in challenging market conditions.
  10. Maintaining positive relationships with landlords of private and society operated affordable housing, keeping up to date on availability of long term housing
  11. Maintaining accurate, up to date and appropriate records and ensuring all data is current and captured in relevant databases 
  12. Completes and maintains current files, case notes, daily intakes and relevant client information
  13. Completes agency required staffing forms such as time sheets, leave requests and expense claims
  14. Performs other related duties as required

Minimum Qualifications

  • Bachelors degree or diploma in Social Work, Psychology or other Social Sciences related discipline
  • 1+ years recent related work experience with vulnerable populations
  • Life experience may be considered in part for related experience
  • OR - An equivalent combination of education, training and experience relevant to the level of work
  • A valid BC driver’s license and access to personal vehicle for business related purposes
  • First Aid, NVCI certification an asset
