Blood Ties Four Directions Centre is seeking a:

Housing Coordinator & Case Manager
(full-time, 35 hours per week)

Position Overview: Housing & Homelessness Coordination and Case Management

The goal of the Housing Program is to reduce the risk of housing loss, housing crisis, and housing instability for populations that Blood Ties serves.


The Housing Coordinator and Case Manager works collaboratively with other agency staff as well as community partners to deliver the following activities:


  • Provide housing transition support planning and case management for BT members exiting WCC, Detox, Drug Treatment programs
  • Provide housing navigation services: Assist clients in finding safe and affordable housing through: accompaniments to view apartments; skills & support in house hunting and talking with prospective landlords; liaising with social service workers; and assisting clients in exploring their housing options
  • Provide housing loss prevention through advocacy and working within an eviction prevention model
  • Provide case management and support for clients with the goals of: helping clients transition into long-term stable housing; helping client build life skills around housing management; achieving life skills
  • Provide case management centred around skill building intended to alleviate the risk of food insecurity, incarceration, addiction, poverty, and poor health
  • Provide housing education programs
  • Facilitate access to the Emergency Housing Fund

You Have

  • Post-Secondary degree in social work, human services, criminology, psychology, community development, or Indigenous studies fields
  • Experience in delivering coordination and/or support services to vulnerable populations
  • Experience providing advocacy services
  • Knowledge or experience in case management practices to vulnerable populations
  • Ability to work collaboratively and build partnerships
  • Flexible, positive and solution oriented approach to work
  • A strong commitment to client service and the ability to respond sensitively to a diverse client group with varied needs
  • An understanding of issues around homelessness, harm reduction, and the social determinants of health
  • Cultural competence in relation to First Nation people
  • A valid driver’s license
