Communitas Supportive Care Society is a faith-based, registered charity providing care in communities across British Columbia to those living with disabilities. As a part of our mission, we provide services ranging from 24-hour residential care to skills-based day programs to respite care for families. At Communitas, we support and empower people of all abilities, from all walks of life, regardless of faith, social standing, race or ethnicity.

 

This permanent full time, 40hr/week position is responsible for the development, coordination and evaluation of all activities relating to the day-to-day care of the residents at First Avenue. The Manager oversees implementation of policy and procedures for the program. The Manager provides direction to staff according to the Communitas Vision, Mission and Values as well as HR and Program Policy manuals. The Manager will ensure that the living environment is safe and healthy, and work collaboratively with all partners (including Mental Health & Assisted Living) and clients to ensure that residents will experience increased self-management skills in their daily living.

 

SPECIFIC DUTIES

  1. Staff Management
    • Determines appropriate staffing levels for program; interviews and selects candidates for vacant/new positions
    • Chairs monthly staff meetings; facilitates an excellent communication process with First Avenue staff
    • Ensures staff in-services and continuing education programs are provided as necessary
    • Ensures a living and working environment that exemplifies Communitas’ philosophy and values, is motivated to meet the goals of the program, promotes effective communication between all partners, and promotes collaborative problem-solving within the program
    • Provides leadership and training to Mental Health Workers (e.g. medication administration) and ensures a high standard of client care at all times
  2. Administrative Duties
    • Develops documentation and reporting systems to maintain accurate records and communication ensuring completion and timely submission of client related reports\
    • Ensures all equipment is maintained and in proper working order; reports concerns to Program Director; ensures nursing equipment and designated work areas are kept clean and tidy
    • Ensures that building is maintained and is a welcoming environment, in accordance with all required H&S Standards & Regulations
    • Ensures that residents’ supplies/medications are ordered, received and stored as required
    • Liaises with CMHC, Assisted Living, professional organizations, and other community agencies
    • Participates in quality improvement programs
  3. Resident/Client Care
    • Monitors goals, outcomes and levels of care and service provision in the home in accordance with PSR principles
    • Ensures client processes, including intake, planning (Recovery Plan), discharges and transfers of residents, are completed
    • Coordinates the provision of appropriate care services, in cooperation with CMHC
    • Gives oversight to care/recovery planning, meets with Case Manager to review, and liaises with residents’ families as necessary
    • Advocates on behalf of residents; ensures that residents’ rights of privacy and confidentiality are maintained
    • Administers medications, medical treatments and documented procedures as per provincial regulations and according to facility policies and procedures
    • Uses established infection control techniques as required to prevent and control the spread of infection
    • Evaluates the effectiveness of care plans (addressing physical, mental, social, spiritual and learning needs) and treatments, and follows up regarding significant changes in the condition of the resident as per program policy and procedure
  4. Attends approved work-related training as required to promote and maintain professional development
  5. Available by cell phone 24/7, unless otherwise designated
  6. Other duties as required

REQUIREMENTS

  • Experience in the field of Psychosocial Rehabilitation/Psychology/Community Mental Health; ability to implement principles and foster a belief in the recovery process.
  • Training/experience in the safe use of psychotropic medications
  • A philosophy of service that is in line with Communitas’ Vision, Mission and Values
  • Familiar with and able to implement ‘Spirit of Gentleness’ principles
  • High energy and highly motivated with strong interpersonal, organizational, leadership and communication skills
  • Advanced levels of written and verbal communication and conflict management skills, using an Honest, Direct and Respectful approach with the ability to be motivational and positive in interactions with residents and staff
  • Ability to relate well to the family/public/constituency and represent Communitas in a positive and professional manner to the community, outside organizations and government agencies
  • Ability to observe, record and evaluate change in human behavior
  • Ability to model appropriate behaviour (conduct, performance, concern for people, morale, compliance with program policy and procedures, work performance standards, appearance, professionalism, ethics and personal integrity)
  • Ability to work as a member of the team as well as being able to work independently without supervision
  • Ability to take initiative with creativity
  • Ability to cope in emergency situations and a willingness to provide crisis response after hours
  • Possession of a valid Class 4 BC drivers license (unrestricted)Use of personal vehicle required for client appointments, etc.
  • Possession of a valid Emergency First Aid Certificate with CPR
