Carefirst Transitional Care Centre is a newly established 30-bed facility providing short stay respite care service, aiming to create an environment promoting exceptional recovery and simultaneously enriching the quality of life for individuals who are in needs.  Join Carefirst and work with a team of passionate and dedicated health care professionals.

 

Responsibilities:

  • Be responsible for the planning, delivery and management of Transitional Care Centre
  • Provide clinical leadership and supervision of team members in day-to-day delivery of transitional care
  • Ensure smooth operation of the actual delivery of transitional care services and align quality of care
  • In charge of screening applicants for admission and work closely with hospital
  • Provide client-centered care through administration of comprehensive client needs assessment, ongoing case management and care coordination jointly with inter-professional team
  • Prepare progress and evaluation reports on services and make recommendations for program development and improvement

 

Job Requirements

  • Completion of recognized primary care or nurse practitioner program
  • Master of nursing; current extended class certification with College of Nurses of Ontario
  • Minimum 5 years relevant clinical experience in primary health care setting
  • Sound working knowledge of nursing function, chronic disease management and geriatric care
  • Ability to deal with crisis situation promptly and effectively
  • Strong interpersonal, leadership and problem solving skill
  • Outstanding clinical assessment skills with good judgment and instinct in understanding clients’ needs
  • Proficient computer skills in Microsoft Office, InterRAI Cha and Practice Solutions
  • Valid CPR and First Aid certification
  • Fluent in English, and a second language commonly used by our clients such as Cantonese is an asset
  • Current vulnerable sector check (within one year)
