About The Project:

The objectives of HIV and Hepatitis C Intervention Project are 1) to increase knowledge and promote the prevention of HIV and Hepatitis C among at-risk immigrant populations, 2) to increase HIV and Hepatitis C knowledge of settlement/community workers, 3) to encourage testing of HIV and Hepatitis C among at-risk immigrant populations, and 3) to follow up and link infected persons to care facilities.

 

About the Position:

The Health Education Facilitator is to support the Project Manager to reach the target clients and implement the HIV & Hepatitis C Intervention Project by performing project outreach among at-risk immigrant populations in the Lower Mainland, recruiting clients for workshops, facilitating HIV & Hepatitis C Education workshops, and performing clients’ follow-up and executing project administrative duties.

Reports to: Project Manager

 

Key Duties and Responsibilities:

  • Assist Project Manager to work with partners and stakeholders to conduct outreach activities to promote HIV & Hepatitis C health education that target ethnic groups from members of LGBTQ2+, homelessness, multiple addictions, and at-risk immigrant populations
  • Outreach to targeted clientele by participating in community events, exhibits, etc.
  • Deliver multilingual HIV & Hepatitis C health education workshops targeting HIV & Hepatitis C at-risk communities in the Lower Mainland
  • Train settlement/community workers on HIV & Hepatitis C knowledge and follow the protocol for referral process
  • Refer participants through a cascade-of-care roadmap, and accept clients into social services to deal with social determinants of health in order to maximize treatment adherence and health outcomes
  • Administer, collect and compile project evaluation and feedback from participants in workshops and follow-up sessions
  • Implement the following-up of clients by phone, email, etc.
  • Explain medical services available to target audiences, including addressing audience’s special needs or concerns
  • Assist the Project Manager in continuous refinement of a culturally appropriate and language-specific HIV & Hepatitis C education curricula over the course of the project
  • Update Project Manager on project progress on a regular basis
  • Maintain accurate service records; compile monthly statistics and reports
  • Perform minutes taking in staff meetings
  • Translate project materials as needed
  • Other project-related duties as assigned by the Project Manager, Program Director or designate

Qualifications and Experience:

  • Academic degree in community health sciences, social epidemiology, communications or equivalent
  • A combined 2 years or more of practical experience in public health education, BC health care environment, community engagement and settlement services, including project promotion and implementation
  • Knowledge in HIV and Hepatitis C Education a definite asset
  • Experience in delivery of public health education
  • Knowledge of immigrant settlement services and BC health system an asset

Job Skills and Abilities:

  • Skillful in group facilitation
  • Proficient verbal and written communication – fluency in English, plus fluency in at least one non-English language for service delivery in immigrant communities
  • Excellent computer skills in MS Office, Outlook, Internet, E-mail
  • Practical knowledge in utilizing various social media channels
  • Effective interpersonal skills and a team player

Other Requirements:

  • Require to travel within the Lower Mainland
  • Willing, ready and able to work at different service delivery sites
  • May be required to work on weekends, evenings
  • Clear criminal record check
