Location/Lieu: Burlington, Ontario

Position/Poste: Community Case Coordinator – in partnership with Halton Housing
  • Assist with case management and service coordination by proactively identifying an individual’s needs and providing assistance with referrals to gain access to social and health services and education in areas such as well-being, health promotion; facilitation of providers to deliver support services in-house or through external agencies. 
  • Responsible for the coordination of programming and service delivery for programs such as friendly visiting, security checks, exercise programs, assisted shopping, social and recreation activities and health promotion inservices. 
  • Responsible for the recruitment of Volunteers to assist and support programs being offered within the buildings.
  • Provide managerial oversight to the implementation and maintenance of volunteer database and statistical record systems. 
  • Create and implement a formal and informal volunteer recognition program. Manage the development and distribution of volunteer materials and resources. 
  • Responsible for the development, implementation and evaluation of the annual program plans including the risk analysis, establishment of key performance indicators, and preparation and management of the program budget. 
  • Accountable for working collaboratively with community agencies at 410 John Street and in the surrounding areas of Burlington to ensure the client’s needs are met.
Qualifications/Compétences requises
  • Post secondary education, Social Services/Volunteer Development/Volunteer Management Diploma or designation preferred
  • Minimum 5 years related managerial experience that includes the following: 
    • Demonstrated volunteer leadership/administration experience
    • Demonstrated supervisory and people management skills
    • Demonstrated program development, implementation and evaluation skills
    • ​Demonstrated experience in budget planning and financial management 
  • Demonstrated knowledge in the areas of volunteer recruitment, orientation, training, recognition and relationship management best practices. 
  • Proven expertise in volunteer resources development and volunteer management  
  • Demonstrated leadership abilities
  • Experience and comfort using Microsoft Office products, particularly Word, Excel and Power Point
  • Excellent interpersonal communication skills
  • Good command of the English language both verbally and in writing
  • Ability to work effectively as part of a team and independently
  • Flexibility to changing demands of the job, including non-traditional hours required
  • Strong organizational skills, ability to prioritize multiple tasks and deadlines
  • Driver’s license and access to a reliable vehicle required
  • Bilingualism an asset
Salary Range/Échelle des salaires: $45,000 to $49,000
