Cota is an accredited community-based organization that has been supporting adults with mental health and cognitive challenges to live well within their communities for over 40 years. We provide person-centred supports that assist individuals to find inspiration and hope in their inherent strengths and support them to pursue meaningful change in their lives, as they define it. We also work collaboratively with our funders, our partners and other stakeholders, across the provincial health and social service systems, to inspire positive change in the way that services are delivered to individuals living with mental health and cognitive challenges.


We are seeking a dynamic and innovative leader who will work enthusiastically to support the delivery of recovery oriented, client and family centered and comprehensive services that meet client needs, ensures client safety and works within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services. Client and family centered care is guiding principle at Cota that helps improve decision making processes, health outcome and client experiences. The successful leader will oversee their portfolio in accordance with this guiding principle.


The Program Manager of Community Health Services is accountable for providing leadership in the development and delivery of high quality recovery oriented and client and family centred services at Cota.  This position is responsible for a portfolio which includes, but may not be limited to, managing an Assertive Housing First Team (AHFT).

The AHFT utilizes the interdisciplinary team approach of an Assertive Community Treatment Team (ACTT) to provide Housing First support to individuals who are homeless and living with mental health challenges. A portion of the team’s service capacity will be dedicated to individuals who also are involved with a mental health court diversion program and/or are on release from a provincial correctional facility.


The pairing of rent supplemented housing and intensive wrap around support is intended to assist individuals to live safely and successfully in the community of their choice, achieve improved personal health and wellness, and to reduce pressures on the criminal justice system.


About the Position:


The Program Manager is responsible for the daily operations and clinical supervision of this interdisciplinary team. This team provides housing opportunities for clients using a “housing first” scattered site approach. Self-determination, choice and harm reduction are at the centre of all considerations with respect to housing and support services.


Summary of Duties:

  • Hire, train, supervise and provide administrative and clinical support to frontline staff
  • Establish new and ongoing positive relationships with partner agencies and external stakeholders
  • Identify and implement quality improvement initiatives as required
  • Oversee management of program budgets with internal support
  • Play a lead role in developing/maintaining and providing education of program specific operating standards / guideline manuals
  • Act as a representative of Cota at various network and/or external committee meetings
  • Participate as a member of the Clinical Leadership Team in planning / developing of the team’s goals and objectives as well as participating in a number of organization-wide activities
  • Act as a member of the Cota Leadership Team
  • Report directly to Director, Community Health Services

Summary of Qualifications:

  • Graduate of a recognized University Program in a related health care or social service discipline
  • Current registration in good standing with the respective professional college (e.g., OT, SW, RN, etc.) will be considered an asset
  • Excellent clinical skills with at least 5 years’ experience providing community-based treatment and services to homeless populations with serious mental illness, justice involvement and complex health conditions
  • One year supervisory or program management experience with a multidisciplinary team
  • Demonstrated excellence in organizational, interpersonal and communication skills, team building and coaching abilities are essential
  • Experience in hiring, training, supervising and retaining qualified staff
  • Knowledge of Housing First, Intensive Case Management models are required
  • Strong knowledge of evidence-based practices used in the mental health and addiction field including recovery practices and harm reduction
  • Solid team player with a demonstrated ability to work with flexibility and efficiency in a fast-paced environment
  • Ability to work effectively with individuals with complex needs and multi-system involvement
  • Ability to maintain a positive, solution-focused approach when responding to conflicts or problems
  • Experience understanding and managing fiscal aspects of the program
  • Proven cultural competence and ability to work effectively with diversity
  • Knowledge of information management, quality improvement and evaluation activities
  • Experience in networking and working in partnership with other groups/agencies
  • Experience working in a unionized environment in a supervisory capacity will be considered an asset
  • Familiarity with Anzer electronic health record, OCAN and OPOC reporting will be considered and asset
  • Working knowledge in Microsoft Office – Word, PowerPoint, Excel, Outlook
  • Ability to go above and beyond and demonstrate initiative

Work conditions:

  • Generally Monday through Friday
  • Ability to work weekends / evening hours
  • Rotating on-call responsibilities

Salary Range: $70,000-75,000
