Job Summary

Black Creek CHC is a community-based primary care organization that is committed to addressing the social determinants of health in a client-centered, equity driven manner.  Working within the inter-professional Chronic Disease program, the Exercise Specialist will provide exercise education and counseling for adults at risk of and living with COPD. In addition, the Exercise Specialist will support the development and implementation of exercise programs for other areas of the organization.



  • Conduct fitness assessment of clients, including strength and aerobic capacity
  • Provide individual exercise prescription, counselling, and supervision of clients within scope of practice taking into consideration ethno-cultural backgrounds and the social determinants of health
  • In close collaboration with the Respiratory Therapist and Registered Kinesiologist in the Diabetes Education program, develop client-focused health maintenance plans utilizing a holistic and inter-disciplinary approach
  • Ensure continuity of care by maintaining up to-date medical records, communication with the client’s primary care provider and other service providers, and making referrals where appropriate
  • Use a variety of teaching methods and principles of education and behaviour change to engage with clients through various exercise modalities
  • Design and facilitate group exercise classes and health and wellness workshops
  • Engage in health promotion, outreach activities and exercise education in the community
  • Advocate on behalf of clients with respect to systemic issues as they arise
  • Participate in program planning, evaluation and resourceful development for the Exercise Education program based on a client-centered, inter-disciplinary approach
  • Act as an exercise resource to staff members and community stakeholders



  • Undergraduate degree in Kinesiology or Physical Education from a recognized university and/or college
  • Some experience working with, and prescribing exercise for clients; specifically those dealing with chronic diseases such as COPD
  • Registration with the College of Kinesiologist of Ontario and/or CSEP-CEP Certification is an asset
  • Experience in program development, implementation, monitoring and evaluation
  • Experience in group facilitation and utilizing various adult education methods
  • Understanding and demonstrated ability to work effectively in a collaborative, inter-disciplinary team environment
  • Excellent organizational, communication and interpersonal skills, and demonstrated ability to take initiative
  • Experience working in cultural, racial, economic, and socially diverse communities and demonstrated commitment to anti-oppressive practices
  • Experience in a community health setting; an understanding and experience with the Community Health Centre model of care is considered an asset
  • Experience in the use of computers, various software applications and EMR systems such as Nightingale on Demand (NOD)
  • Current CPR C + AED certification
  • Proficiency in a second language that is represented in BCCHC’s catchment area considered an asset
  • Must be available to work a flexible schedule, including evenings and weekends
