The BC Centre for Excellence in HIV/AIDS’ Downtown Eastside Research and Innovation Centre a unique and innovative health centre comprised of a multidisciplinary centreal team focused on providing comprehensive care for patients/clients in the Downtown Eastside.  The high-quality care provided by the centreal team focuses on HIV/AIDS, HCV and other infectious diseases and is supported and enhanced by ongoing research, innovation and wellness promotion.

The Registered Nurse (Primary Care) is a member of an interprofessional health care team.  Duties include working with the health care team to assess and engage new patients/clients, documenting assessments and developing care plans and interventions in consultation with team members.  In collaboration with team members, the Registered Nurse (Primary Care) participates in and facilitates team and case management meetings.

 

Duties:

  • Use chronic disease and health management and/or self-care management principles to conduct assessments, identify problems, address issues or variances from the plan of care in collaboration with the appropriate physician and/or interdisciplinary team member.
  • Triage for primary care (e.g. respiratory, gastro-intestinal, diabetes) and psychosocial needs.
  • Perform primary care nursing duties as they relate to wound care, addiction assessments, harm reduction for sexual health and addiction, HIV and HCV point of care testing, phlebotomy and PAP smears, diabetes care, pain management, STI screening, and pregnancy and urine drug tests.
  • Provide continuity of care by liaising with interdisciplinary team members, implementing centreal interventions, evaluating patient/client outcomes based on patient/client goals for care, and coordinating ongoing care and supporting transition.
  • Act as a resource for the interdisciplinary team by providing information based on nursing theory and practice related to patients/clients with complex health care needs.
  • Consult, confer, and collaborate with multidisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in patient/client-centred education and research.
  • Perform other duties as assigned.

Qualifications:

  • Registration with the BC College of Nursing Professionals (BCCNP) as a practicing RN registrant.
  • Two (2) years’ recent related experience working with diverse marginalized populations in health care settings or community in a primary care, HIV, addictions and/or mental health setting, and previous experience working with an interdisciplinary team.  Certified Practice for STI care, Narcan Certification, CPR Certification and completion of vaccines courses required.

Skills and Abilities:

  • Demonstrated knowledge and experience within a patient/client centred model of care.
  • Comprehensive triaging skills for primary care (e.g. respiratory, gastro-intestinal, diabetes) and psychosocial needs.
  • Knowledge of nursing standardized tests and measures.
  • Demonstrated skill in physical assessments and chronic disease management.
  • Demonstrated competency in psychomotor skills required within the designated scope of nursing practice.
  • Demonstrated knowledge of wound care, addiction assessments, harm reduction, HIV and HCV testing, phlebotomy and PAP smears, diabetes care, pain management, STI screening, and pregnancy and urine drug tests.
  • Knowledge of and exposure to trauma-informed care, violence prevention, transgender care, harm reduction, recovery-oriented practice and indigenous cultural competency training.
  • Knowledge of chronic disease management principles.
  • Knowledge of self-management principles and approach to care.
  • Knowledge of medication reconciliation.
  • Demonstrated ability to communicate effectively verbally, including using funneling and bridging statements when addressing sensitive areas of care and open and closed questions to obtain a clear history and maintain strong patient/client relationships.
  • Ability to role model and employ effective conflict resolution, de-escalation and reconciliation approaches and techniques.
  • Ability to work collaboratively as a member of an interdisciplinary team. 
  • Demonstrated knowledge of patient/client-centered education principles, methods and tools as it relates to healthcare needs.
  • Demonstrated knowledge and ability to perform CPR.
  • Computer literacy to operate a computerized patient care information system and email system.
  • Ability to operate related equipment.
  • Physical ability to perform the duties of the position.

STATUS/HOURS: The hours of work at the centre will begin on a Monday to Friday 10:00 am to 6:00 pm basis and grow into an extended hour, 7 day a week operation. We can offer various shift rotation opportunities including full-time and part-time work. This is a Research and Innovation Centre and therefore we will measure the operation prior to changing the hours.

START DATE: August – September 2019

LOCATION: 625 Powell Street, BC Centre for Excellence in HIV/AIDS, Vancouver, BC, and various other local BC-CfE sites as required
