Location / Lieu: 75 Cooperage St. Toronto – Acquired Brain Injury Services

Position / Poste: ABI Rehabilitation Worker – Permanent Part Time
Duties / Fonctions:
  • Assist consumers to gain greater autonomy in activities of daily living such as, personal hygiene cognitive
  • and problem solving skills, support with communication skills, community orientation/ integration while
  • supporting and enhancing the overall improvement of quality of life
  • Reporting responsibilities, i.e. progress notes, Incident reports, data collection and filing
  • Ongoing assessment of client’s relevant skill strengths and needs
  • Develop, coordinate and implement the delivery of specific services within the community
  • Liaise with other social service agencies and health care providers involved with consumers
  • Participate and provide training as required
  • Will be required to manage and schedule caseloads
  • To provide services on a flexible basis including days, evenings, weekends, and holidays.
Qualifications / Compétences requises:
  • Post- Secondary Education Social Services or equivalent education and experience.
  • Previous experience working with ABI consumers and rehabilitation is an asset.
  • Current CPI and CPR, First Aid an asset.
  • Ability to assess consumer needs, translate this into a service plan & establish/maintain appropriate
  • controls
  • Experience with Microsoft Office
  • Good interpersonal, organizational and communication skills, including de-escalation techniques
  • Professionalism in dealing with other service providers
  • Demonstrated team building skills
  • Sound behavioural management skills, cognitive problem solving skills.
  • Ability to speak, read, and write English.
  • Ability to work every other weekend
