Position / Poste: Coordinator, Warmline and Community Outreach
 
Duties / Fonctions: 
  • To provide a customer service approach when responding to telephone and online inquiries received from Stroke Survivors, Polio Survivors, Caregivers, families and Health Care Professionals. 
  • A key area of responsibility is the maintenance of the Post-Polio and Stroke Recovery registries, databases, mailing lists, interactive websites and informational resources. 
  • To provide support and development in community outreach for After Stroke and Post-Polio. 
  • The role requires excellent customer service and organizational skills as well as knowledge of database management 
  • Regular report creation and trend monitoring will be important in this position to ensure that department/program objectives meet the needs noted by our consumers. 
  • Excellent internet research skills are essential to the fulfillment of this job as consumer inquiries can often pose unique questions. 
  • The Coordinator will also be required to follow-up with consumers to ensure that any information sent (via electronic or paper form or voicemail messages) was received, understood and of value to the consumer. 
  • The incumbent requires excellent communication and organizational skills as they work independently and as a member of a team, and are required to work directly with a wide range of internal and external contacts in fulfillment of their job. 
  • The Coordinator is responsible for creating/updating and communicating policies and procedures related to the areas of expertise they work in. The incumbent must understand and be able to effectively communicate how their work interplays with other areas of the department/program as well as the organization and the external environment
Qualifications / Compétences requises: 
  • Post-secondary degree in a related area or equivalent experience – Social Services, Health Promotion,Recreation Therapy, Social work 
  • Minimum two years professional experience 
  • Demonstrated ability to work with community agencies, stakeholders and the public. 
  • Caring and compassionate by nature 
  • Demonstrated knowledge in working with individuals living with a chronic health condition 
  • Excellent interpersonal, communication, presentation, public speaking and diplomacy skills. 
  • Demonstrated organization skills and the ability to effectively multi-task and triage. 
  • Understanding and respect for diversity and those with disabilities. 
  • Proficiency in report writing and data collection. 
  • Proficiency in Windows based software (Word, Excel and PowerPoint). 
  • Driver’s license and access to a reliable vehicle required.
Salary Range: $45 000.00 annually
