Youth and Family Development Worker – MAT LEAVE (Upon Return of Incumbent)

Job Summary:

 

The Youth and Family Development Worker provides assessment, individual case planning, and implementation of services for youth and families referred to the program utilizing an Integrated Case Management Model.

The Youth and Family Development Worker assists the youth and/or family to create case plans by identifying strengths, gathering information and setting and achieving realistic goals and investigating educational or employment opportunities.

 

Along with other employees of the Society, the person appointed to this position will adhere to the Policies and Procedures Manual.

 

Reports To:

 

The Youth and Family Development Worker is responsible to the Co-ordinator: Youth Programs, or the assigned designate. The Youth and Family Development Worker may report, when appropriate or in the event of an emergency, to the Executive Director.

 

Key Duties and Responsibilities: 

  • Interviews youth and families to prepare case histories, assess strengths, prepare case plans, deliver a range of services, and evaluate interventions.
  • Develops and implements behavior intervention programs to assist youth and families in determining and attaining goals. Plans, prepares and conducts individual or group support sessions with youth and families.
  • Monitors youth and/or family’s progress and well-being. Provides suggestions for the modification of client’s goals, program and/or plans.
  • Evaluates the effectiveness of services in the context of the case management team. Resolves identified problems and moves towards defined objectives. Follows up results of services and the youth’s achievements with other service providers and the case management team.
  • Recognizes potential crisis situations, analyzes such situations accurately, develops strategies to deal with such situations, involves other service providers as needed including emergency services, and informs the supervisor when such incidents arise.
  • Provides life skills training. Teaches youth effective social and interpersonal skills through tasks of daily living and social activities.
  • Acts as an advocate for the youth, provides support for youth while they are receiving service and, as appropriate, provides follow up support to them when they leave the program. May accompany the youth to appointments.
  • Provides emotional support and feedback to youth and families. Assists youth and families to attend appointments and provides transportation when necessary.
  • Provides monitoring and support services to youth on probation in accordance with the youth’s probation order.
  • Develops Community Work Service placement sites and monitors youth on Community Work Service orders.
  • Adheres to required levels of file maintenance. Maintains necessary reports and records.  Ensures that all required documentation is accurate and complete.
  • Liaises with other service providers when appropriate.
  • Maintains a thorough knowledge of the current research, best practices, and community resources to meet the needs of youth.
  • Participates in evaluating the effectiveness of the program. Provides feedback regarding possible modifications to the program to the direct supervisor.
  • Performs other related duties as required.

QUALIFICATIONS

Education, Training and Experience:

  • A Bachelors degree in social work, psychology, counselling or a related field, or the equivalent combination of education and experience.
  • One to three years previous work in a similar environment required. Direct program delivery experience in the community social services sector with a demonstrated working knowledge of community-based programs and related provincial and community support systems.
  • To be familiar with and adhere to the B.C. Association of Social Workers: Code of Ethics as well as other relevant legislation and ethical codes.

  • Well-developed interpersonal, oral and written communication skills.
  • Assessment skills.
  • Proficient teaching skills.
  • Good organizational and time management skills.
  • Knowledge of group process and facilitation techniques.
  • Ability to work effectively with other agency staff, therapists, social workers, and other service providers.

 

Additional Information:

 

  • This position is required to work in an exciting and busy environment often dealing with youth in crisis situations brought about by family challenges. The ability to function independently and frequently in a busy environment while managing multiple services, including crisis intervention, is an ongoing expectation of this position.  Direct delivery of services may require a moderate level of physical fitness, e.g. walking, standing, bending, and lifting to effectively carry out the duties of the position.
  • The Youth and Family Development Worker is expected to work outside the office (e.g. in Ministry offices, clients’ homes, the community, or schools).
  • This position requires the use of a vehicle.
  • A valid Class IV Driver’s license is desirable.
  • As a condition of employment the person holding this position must retain current Level One First Aid certification.
  • As a condition of employment all employees must pass a Criminal Record check.
