POSITION: Social Worker - Veteran Family Program Coordinator
REPORTS TO:Mental Health Team Lead
CATEGORY: Full-time (37.5 hrs per week)
RATE OF PAY: Between $60,229 - $65,144 based on experience
SUMMARY:
Reporting to the Mental Health Team Lead, the Social Worker - Veteran Family Program (VFP) Coordinator will be responsible for the
development, implementation and evaluation of programs and services that will assist CAF members, Veterans, and their families during the
various stages of military life including the pre-release and post-release of a medical release, and the transition to civilian supports and
services. The Veteran Family Program Coordinator helps to determine the needs of the VFP clients and acts as a bridge between the
transitioning family, DND, VAC, civilian support services along with other relevant service providers. This position also supports the Mental
Health and Wellness Team with supporting military families who are not currently experiencing a transition.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Referral and Navigational Support:
• Assist medically releasing CAF members, medically released Veterans and their families in their navigation of, and access to,
relevant community programs and services.
• Provide screening and assessment of all general requests for assistance to determine which program/service would best respond
to the client’s request.
• Provide emotional, informational, and tangible support to families and refer to appropriate professionals and services.
• Provide follow-up with families to ensure referrals have been utilized and are meeting the family’s needs and provide ongoing
support to families as required.
Client Services:
• Establish contact with medically releasing CAF members, medically released Veterans and their families to provide a general
overview of the services of the VFP and MFRC.
• Conduct intake meetings with family members to assess their situation and need for assistance.
• Provide emotional, informational, and tangible support to families and refer to the appropriate professionals and services.
• Employ a method of follow-up with families to ensure referrals have been utilized and are meeting family’s needs.
• Provide information and referral services for the support of family members in crisis or imminent crisis.
• Act as a community service Subject Matter Expert (SME) for families with intricate needs.
• Assist families in their navigation of military/civilian systems to access relevant programs and services.
Community Engagement:
• Develop and maintain partnerships with community groups, support agencies, schools, and other organizations to enhance service
delivery to CAF families.
• Develop networking opportunities within the military community, other MFRCs, and the larger community as required to create and
maintain a network of resources and referrals for families.
• Review, compile and maintain a list of resources and services that meet military families’ unique needs.
• Actively engage with stakeholders, through participation on committees and working groups.
• Provide community organizations and service providers with up-to-date information about MFRC.
• Maintain an information-sharing network with MFRC programs, military resources (social workers, padres, etc), and maintain open
communication with VFP and the other Veteran Family Program. Coordinators to ensure continuity of service provided across all
MFRC’s.
• Keep up to date on trends, initiatives, and policies in the field of support for military families; transfer that knowledge into daily
practices/initiatives.
Monitoring and Evaluation:
• Evaluate services formally and informally on an ongoing basis.
• Promote evaluation of the VFP program through monthly survey calls and electronic banners provided by VFP headquarters.
• Collect and report on statistics and demographic information that ensures client confidentiality.
• Evaluate services both formally and informally on an ongoing basis that is compatible with the MFRC’s overall evaluation plan.
Program Development and Promotion:
• Develop and implement the Veteran Family Program plan in concert with MFS logic model
• Identify, develop, and implement, under the direction of the Mental Health Team Lead, preventative workshops and seminars on
interpersonal relationships, family violence, general well-being, and family lifestyle stressors. Workshops and seminars may take
place in the evening and weekends to meet client needs.
• Collect and report on statistics and demographic information in a way that ensures client confidentiality, in accordance with the
MFRC’s agency statistical systems.
• Ensure all client services and programming follow evidence-based practices.
• Provide or support the development of awareness sessions, briefings and resources pertaining to change management and
transitioning into a civilian community actively promote MFRC programs, activities, and services.
• In collaboration with the MFRC communications departments, actively promote VFP programs, resources and services by such
means as making public appearances, circulating pamphlets and posters.
• Provide general information briefings, attend briefings and preparing articles for regular submissions to the marketing and
promotions area.
• Execute outreach to ensure members and families are receiving information on services and programs in a timely manner in
person and/or virtually.
Other:
• Ensure agency policies and procedures are adhered too.
• Provide monthly reports, evaluation findings and statistical data.
• Consider and alert to management any risk management issues related to program area.
• Facilitate Emergency Family Care Assistance (EFCA) program when required
• Provide information and briefings about the Centre to individuals, groups, and organizations.
• Attend and participate in meetings, committee work, and fundraising events as required.
• Undertake any secondary tasking and responsibilities deemed necessary for the efficient operation of the organization, its
programs, and services.

standards.

• Handle confidential information with professionalism and sensitivity.
DESIRED SKILLS/ABILITIES, KNOWLEDGE, EXPERIENCE AND EDUCATION:
• Degree or diploma in social work with membership or the ability to register with the Alberta College of Social Workers (ASCW).
• Several years of experience in providing human services frontline support.
• Several years of experience in planning, implementing and evaluating programs.
• Successful completion of a clear Vulnerable Sector Criminal Records Check, clear Child Welfare Intervention Check and Enhanced
Reliability Screening.
• A willingness and ability to work flexible hours, including evenings and weekends on occasion.
• Strong facilitation and communication skills that actively engage clients.
• Knowledge of the unique challenges of the military lifestyle
