Job Summary:

 

The Case Manager helps youth reduce barriers and connect them to employment opportunities that enhance their self-sufficiency in the labor market.

 

Reports to: Program Coordinator

 

Key Duties & Responsibilities:

 

  • Markets the program to referring agencies, community agencies, and the general public for client referrals.
  • Conducts information sessions with youth on available services and resources.
  •  Conducts intake interviews with youth applicants to assess their eligibility and needs for the project.
  • Conducts needs assessment for each registered client to determine intended career goals and support requirements.
  • Creates an individualized Action Plan with each youth applicant.
  •  Provides intensive Employment Services (ES) cohort training and guidance to prepare youth for Quality Employment Opportunity (QEO - work experience placements).
  • Facilitates, and continually develops dynamic and engaging workshops for youth in the phases of ES and touch-back sessions during QEO.
  • Strengthens youth’s job search skills and employability potential through creating and facilitating confidence-building activities and sessions.
  • Works with Employer Specialists to identify and match a suitable employer for youths.
  • Develops and monitors a QEO plan between youth and employers and specifies the QEO objectives, goals, milestones, and expected outcomes.
  •  Provides ongoing support to both youth and employers to ensure QEO training and development objectives are being met.
  •  Arranges follow-up meetings with youth on a weekly basis to provide consistent support and guidance.
  •  Provide one-on-one support to youth on achieving sustainable employment and/or return to school.
  •  Prepares documentation and reports related to program activities, and tracks participant activities and progress data.
  •  Prepares successful stories of youths in monthly, quarterly, and annual reports.
  • Performs other job-related duties and responsibilities as required.

 

Education. Training & Experience:

 

  • Bachelor’s Degree in Business, Social Sciences, Social Work, Career Development or Vocational Rehabilitation
  •  Certified Career Development Practitioner (CCDP) or equivalent designation
  • Minimum of two years’ work experience in a social service environment as an employment case manager, job developer, employer liaison, or job coach
  • Experience in facilitating workshops/group activities, including but not limited to job search, basic life skills, essential work skills, and soft skills
  •  Experience in connecting with employers to provide job experience for clients
  • Experience in working with vulnerable clients with complex barriers and challenges
  •  Experience and knowledge in career counseling
  • Knowledge of intercultural and social issues
  •  Knowledge of the local labor market information and trends in British Columbia

  • Criminal Record Check Required
  • Required to work at multiple outreach locations across Greater Vancouver
  • Ability to work occasionally on weekends and evenings as required
