Job Position: Client Care Counsellor

Department: Health Services

Status: Full-Time – 35 hours per week

Location: Mississauga & Brampton

Compensation: $26.83 per hour plus group health and RRSP benefits

Position Summary:

Under the guidance of the Manager, the incumbent is responsible for administrative supports to the delivery of services including managing new inquiries, completing assessments, enrollments, scheduling and linking program participants to appropriate community resources in a culturally sensitive manner, and ongoing monitoring of needs and discharge planning, offering language specific communication in one or more South Asian language. They shall demonstrate skills that reflect the organization’s ethics, integrity and excellence standards in promoting a client centered philosophy.

Responsibilities and Duties: (but not limited to)

* Complete suitability assessment including initial intake, new program participant documentation and setup of program participant records in database.

* Develop care and goal plans and other relevant documentation for new and ongoing program participants.

* Complete/update interRAI CHA assessment annually and on an as needed basis, ensuring. data reflects accurate and current program participant information.

* Update electronic health records with all personal data changes and create and maintain a paper-based file of all required documentation according to policy in the event the electronic files are not accessible.

* As the designated Caseworks expert, train, support and ensure program staff are following all required documentation standards. Conduct regular documentation audits to ensure that updates are completed by designated staff.

* Maintain accurate documentation and records within all required databases.

* Participate in regular assessments, documenting program participation and changes to physical, psychological and/or social wellbeing; Notify the relevant staff of any changes to the program participants status.

* Refer ineligible prospective program participants (do not meet program criteria) to other external programs and services.

* Facilitate program participant’s referrals and access to services within the agency and in the community including identifying program participants who could benefit from ancillary Health Services programs and services.

* Responsible for coordinating Security Reassurance services, receiving and implementing requests from Life Enrichment staff, including contact with caregivers.

* Support caregivers with transportation advice to assist with attendance planning.

* Research, develop and maintain current community resources, supports and services for program participants and caregivers.

* Support community events, projects, fairs and outreach activities as needed.

* In conjunction with the Manager:

* Manage extended absences including discussions pertaining to change in location/schedule of service, health condition, transfers, discharge and or exit of program participants with caregiver

* Maintain and manage prospective program participant waitlist to optimize utilization of program spaces

* Develop attendance plans for new and ongoing program participants ensuring appropriate programming matches and distribution of work within the program. Keep administration staff apprised of any program participant scheduling or attendance issues.

* Monitor relevant databases for new referrals, compile monthly stats and analyze results for follow-up action.

* Prepare and submit statistical and progress reports as required by the organization and by the MSAA.

* Conduct regular statistical and feedback analysis and develop summary and recommendations report.

* Lead the Caregiver Support Program by active promotion of the events with caregivers, participate in the program as needed to provide support for identified caregiver needs.

* Assist with problem solving, acting on plans for resolution as directed by the Manager.

* Assist program participants with programming and activities of daily living including personal care as needed.

* Participate in staff and team meetings and lead wrap up meetings.

* Participate in professional development opportunities as required.

* Adopt safe work practices, reporting unsafe conditions immediately.

* Perform other duties as required.

Qualifications and Skills:

* Post-secondary education in Gerontology, Social Work or Social Service Worker, combined with two years of experience with seniors, and/or working in a community health care environment or an equivalent combination of training and experience.

* Demonstrated experience using Inter-RAI CHA and evidence of solid observational skills.

* Knowledge of client-centered case management service delivery.

* Proven ability to listen and make appropriate determinations for support in general service needs.

* Direct experience working with frail, elderly and/or cognitively impaired seniors and knowledge of their unique needs.

* Knowledge of a broad range of social, health and community services available in the GTA.

* Demonstrated organizational and interpersonal skills.

* Self-directed, self-motivated, with a positive attitude and be able to work independently and as part of a team.

* Ability to prioritize, multi-task and adjust to shifting priorities.

* Excellent oral and written English skills are required and one or more South Asian language.

* Advanced proficiency in MS Office, email and internet and database applications.

* Sensitivity to people from different socio-economic backgrounds with an equity lens.

* Valid driver's license and access to a reliable vehicle.

* Ability to work flexible hours including evenings, weekends and to work from any Indus work-site.

* Successful candidates must provide current CPR and First aid certification and a satisfactory Vulnerable Sector Check prior to commencing employment.

* Indus’ mandatory COVID-19 vaccination policy that requires all employees be fully vaccinated by a Health Canada approved COVID-19 vaccine has been temporarily suspended. Currently new employees will not have to show evidence of vaccination. Indus reserves the right to reactivate this policy at its own discretion based on Public Health recommendations.
