Job Title: RAH Hope Medical Navigator

Based at: RAH (Royal Alexandra Hospital) Emergency Department

Reports to: Hope Medical Leadership

Hours of work: Full-time: rotating schedule, four-on-four-off, day or nighttime position

Start date: Immediate

Job Summary:

The RAH Navigator is the primary connector between the Royal Alexandra Hospital (RAH), including the RAH Emergency Department, and Hope Medical at the Herb Jamieson Centre. The role focuses on diverting unhoused individuals from the RAH Emergency Department who do not require hospital-level care to Hope Medical, and on transitioning medically stable unhoused patients from hospital care to Hope Medical for ongoing medical support, social connections, and pathways toward longer-term community care. This is a navigation and connection role, not an intensive case management position.

Major Duties and Responsibilities:

Liaison & Navigation

  • Serve as the main Hope Medical representative within the RAH Emergency Department. 
  • Act as a navigator to ensure appropriate flow of unhoused patients from RAH to Hope Medical.
  • Maintain strong partnerships with hospital staff to support seamless transitions.

Diversion of Non-Emergent Cases

  • Collaborate with the Charge Nurse and ED staff to identify unhoused community members who do not require hospital-level medical care.
  • Facilitate their diversion from the RAH ED to Hope Medical for medical assessment, stabilization, and shelter-linked supports.

Transition of Medically Stable Patients

  • Identify unhoused patients in hospital who are medically stable and suitable for transition to Hope Medical.
  • Support transfers so clients can access ongoing medical care, social supports, and pathways toward long-term or permanent community care.
  • Ensure the focus remains on navigation, coordination, and connection rather than intensive case management.

Transfer Coordination

  • Arrange safe and timely transfers to Hope Medical using Hope Mission transportation teams.
  • Ensure all transitions prioritize dignity, safety, and continuity of care.

Documentation, Reporting & Communication

  • Maintain accurate records, case notes, and statistics on all diversions and transitions.
  • Provide regular updates and clear communication to Hope Medical leadership.
  • Identify trends and opportunities to strengthen hospital-to-Hope pathways.

Additional Duties

  • Support the development and refinement of referral and transfer processes.
  • Participate in training, meetings, and collaborative initiatives with RAH and Hope Medical teams.
  • Uphold Hope Mission’s values of compassion, dignity, and holistic support in all interactions

Key Competencies:

  • Experience working with unhoused individuals in a case management, social work, nursing, or medical support capacity.
  • Strong relational and communication skills with both clinical teams and complex-needs community members.
  • Ability to navigate complex systems and coordinate care across multiple services.
  • Comfort working in a fast-paced hospital environment with diverse and vulnerable populations.
  • Understanding that the role is not intensive case management, but focused on navigation, coordination, and connection.
