March of Dimes Canada (MODC) is a national charity that offers a wide variety of programs and services to Canadians living with disabilities.  Since 1951, March of Dimes Canada has been on the forefront of the disability movement with a mission that is dedicated to maximizing the independence, personal empowerment and community participation of people with disabilities. 
 
The Coordinator, Stroke Services will support stroke survivors and their caregivers to maximize their independence, personal empowerment and community participation through our After Stroke strategy.  These services focus on supporting stroke survivors, their families and caregivers post hospital discharge to successfully return to the home environment and re-engage into their community through emotional support, access to information and required services, and encouragement.  System Navigation, partnership development, volunteer management, knowledge of health care system as it pertains to stroke services are critical components of this role. 
 
Duties:
  • Implementation of a comprehensive volunteer program designed to engage volunteers in leadership and service delivery roles.
  • Strong liaison with health care teams across the region to strategically support and grow our existing Peers Fostering Hope Hospital Visitation program
  • Facilitate community based programs designed to assist with transitioning from hospital/rehabilitation to home and community re-integration
  • Collaborating with local hospitals and the regional networks to develop and implement hospital visitation programs and other initiatives designed to support the implementation of Canadian Stroke Best Practice Recommendations
  • ​​Develop collaborations and partnerships to carry out our After Stroke Strategy for stroke survivors and caregivers
  • Develop, and support peer support and caregiver groups/chapters to provide educational and emotional support in local communities
  • Developing, implementing and evaluating community based programs and volunteer initiatives designed to support people after stroke
  • Work with caregiver networks to provide a broad range of caregiver support across the region
  • Other duties assigned by the Regional Manager, Atlantic Canada
Qualifications:
  • Post-secondary degree in a related area or equivalent experience (Health Promotion, Recreation, Community Development and/or Volunteer Management)
  • Minimum three years of related experience within the fields of  health promotion specifically within stroke community, community development , and volunteer management
  • Experience working with volunteers
  • Proven experience organizing, planning and evaluating events/conferences and workshops
  • Knowledge of principles of adult education
  • Demonstrated research and analytical skills
  • Demonstrates professional, customer-focused behavior
  • Ability to communicate positively and effectively in verbal and written form, excellent command of grammar
  • Demonstrated flexibility to changing demands of the job and multiple tasks and deadlines; results-oriented
  • Self-motivated to achieve results
  • Good organizational and planning skills
  • Quality-oriented in all work produced and services provided
  • Attention to detail and accuracy
  • Demonstrated ability to problem-solve, make recommendations, and take action based on best available information and input from others 
  • Demonstrates willingness and ability to apply new methods and techniques to performance of work
  • Valid driver’s license essential and access to a reliable vehicle
