About the SUCCESS Multi-Level Care Society

 

To care for the seniors like our own by providing a continuum of excellent, culturally appropriate services for them. In 1995, the S.U.C.C.E.S.S. Multi-Level Care Society was incorporated with the purpose of establishing linguistically and culturally appropriate care for those seniors in our community. It has now developed into a provider of quality residential care and health services.

 

POSITION SUMMARY:

 

Under the direction of the HR & Administrative Manager, the Screener is responsible for greeting and engaging with all individuals accessing the care home to ensure their attendance at the care home is essential, communicates established entrance guidelines, and ensures all non-essential visitors follow entrance restrictions. The Screener is also responsible for screening all individuals for signs and symptoms of illness by following the care home guidelines. The position is temporary up to the end of March 2021 and subject to the availability of funding for further extension.

 

 

ROLES AND RESPONSIBILITIES

 

  • Creates a welcoming presence for staff, resident, resident families, contractors and visitors, and using a customer service approach and communicates professionally, courteously and effectively with individuals presenting at the care home entrance and according to the visitation guidelines of care home to identify if their access to the site is essential
  • Screens each individual accessing the site for signs and symptoms of illness by following established and documented processes such as administering symptom screening assessments and questionnaires; and follows established infection prevention and control guidelines.
  • Maintain the screening record and designated screening form.
  • Based on the assessment of the individual determine if the individual can access the site; otherwise, inform the individual to leave the site and report to the DOC/designated.
  • Cultivates a safe and informed environment for individuals with essential access to the care home, by reinforcing and emphasizing established infection prevention protocols, personal protective equipment (PPE) usage and criteria, and proper hand hygiene techniques.
  • Follows current direction and criteria for usage of personal protective equipment (PPE) and for providing PPE to all individuals.
  • Using effective communication skills, explains site access restrictions to individuals, and provides all relevant safety/security and infection prevention and control procedures orientation.
  • Reports safety and security incidents to Screening & Visitation Coordinator/Designated and other site personnel as required and in accordance with established protocols. Documents relevant information on an Incident Report form. 
  • Ensures that all staff and visitors and staff are following safe physical distancing measures in accordance with established visitor guidelines, such as maintaining physical distancing at entrance line-ups while waiting for the screening.
  • Restocks the screening station with PPE supplies, screening assessment forms for the different types of individuals.
  • Performs other related duties as assigned.

 

 REQUIREMENTS:   

 

  • Grade 12 education and two-year work experience or an equivalent combination of education, training and experience.
  • Ability to communicate effectively both verbally and in writing
  • Ability to deal with others effectively
  • Ability to deal with conflicts
  • Physical ability to carry out the duties of the position
  • Ability to organize work
  • Ability to operate related equipment
