POSITION SUMMARY:

 

This position functions as part of a diverse team within a Homeless Shelter/Addictions & Recovery setting. Patients present with various physical health concerns, mental health concerns, addictions as well as other social barriers e.g. homelessness, lack of income. The nurse case manager is a member of the health care team who utilizes the nursing process, critical thinking, problem solving and decision-making skills to provide holistic care to individuals and families.

The nurse assists clients to achieve their optimal physical, emotional, mental, spiritual health and well-being by assessing, coordinating, administering and evaluating the provision of health services and other resources in collaboration with the inter-professional team.

 

 

MAJOR DUTIES AND RESPONSIBILITIES:

  • Provide support to the doctors by addressing concerns that come up during visits that are beyond the doctor’s main responsibilities.
  • Facilitate the development, implementation and ongoing administration of an effective, individualized plan of care in collaboration with the client, the doctor, and other care providers, ensuring the continuity of care across the continuum.
  • Advocate and link clients to other resources as required.
  • Assist clients with completion of documents e.g. AISH applications, treatment facilities, supportive living and other housing applications etc.
  • Provide education for chronic disease management.
  • Provide Naloxone kits and education to patients who request.
  • Provide routine procedures and treatments including but not limited to wound dressings, suture/staple removal, etc.
  • Uses working knowledge of common pharmaceuticals to counsel patients appropriately within a nursing context.
  • Administer injections as prescribed by doctor.
  • Facilitate doctor recommended ER referrals.
  • Provide support and consultation to other Hope Mission staff and/ or clients as needed.
  • Document all interactions with clients.
  • Establish and maintain effective liaison with staff at Hope Mission, other agencies and stakeholders to ensure coordinated services are provided.
  • Monitor and evaluate progress and outcomes.

 

MINIMUM QUALIFICATIONS AND SKILLS 

  • RN Bachelor’s degree or RPN Diploma/Bachelor Degree
  • Valid practice permit from CARNA or CARNPA required
  • Training: First Aid, CPR
  • A minimum of 2 years’ experience in a community or mental health setting preferred

Job Type: Part Time (0.6 FTE)
