Follow-up Support Worker – Intensive Case Management Team

Are you a relationship builder, team player and passionate about supporting individuals in securing
housing options based upon their personal goals? The Intensive Case Management Team is part of the
service delivery team of Homeward Trust Housing First in Edmonton that empowers individuals through
a collaborative exploration of a person’s strengths, barriers, interests and goals. You will build
and maintain trusting partnerships with landlords, community service providers and program
participants in navigating affordable housing options and income funding. You will create individual
service plans, manage a caseload of 17-20 program participants and provide on-going support to
participants through regular home visitation visits.

Hours:  Monday to Friday during office hours for 40 hours per week; requires use of personal vehicle and travel around Edmonton.

Temporarily Remote: This position will be required to work from your home office temporarily through Covid-19 restrictions.

In Your Role You Will

  • Use assessments, referral skills and tools to assist in exploring participant’s strengths and barriers in obtaining sustainable affordable housing options.
  • Build Individual Service Plans based upon participant strengths, interests and goals through exploring their talents and interests.
  • Assist participants in navigating and accessing appropriate income funding available from federal and provincial government sources.
  • Provide on-going assistance to participants’ transitions into the community to access support services related to physical, emotional, intellectual, spiritual and/or mental health needs.
  • Promote e4c Intensive Case Management Team’s services to potential property management organizations or landlords.
  • Support participants with re-housing procedures as required.
  • Maintain positive working relationships with landlords through regular contact and support.
  • Develop written and electronic case notes and documentation using Effort to Outcome software and regularly complete Service Prioritization Decision Assistance Tool (SPDAT) assessments and provincial assessments.
  • Meet competing deadlines with your ability to multi-task while managing multiple priorities.
  • Engage and communicate with a variety of different audiences while building professional and trusting relationships.

Education and Certifications

  • Diploma in Human Services, Social Sciences or related discipline.
  • Police Information Check with Vulnerable Sector Record Check.
  • Intervention Record Check.
  • Certifications (or willingness to obtain): Suicide Intervention, Standard First Aid and CPR, Mental Health First Aid, De-escalation
    Requires a use of personal vehicle with valid Class 5 Driver’s License, Vehicle Registration, Vehicle Insurance (2 million dollar liability) and Driver’s Abstract.


  • 1+ year of experience working with marginalized populations, high risk and vulnerable
  • Previous experience working with LGBTQ2+, Indigenous, refugee and immigrant populations.
  • Working knowledge of harm reduction, abuse and trauma, sexual exploitation, addictions, mental health, homelessness, inner city and indigenous culture.
  • Knowledge and experience of Motivational Interviewing and Critical Time Intervention strategies considered an asset.
  • Knowledge of Alberta’s Residential Tenancy Act and Residential Tenancy Dispute Resolution Service is an asset.
  • Demonstrated experience de-escalating crisis and potentially violent situations with individuals under the influence of drugs or alcohol, under extreme stress &/or struggling with acute/chronic mental health crises.
  • Demonstrated ability to foster positive relationships with a variety of audiences.
  • Ability to multi-task and manage multiple priorities in a home office setting (temporary).
  • Proficient in Microsoft Outlook, Word, Excel, database applications and computer operations.
