The Addictions Case Manager employs a strengths-based and client-centred approach to supporting individuals with addictions and mental health issues.  You collaborate with a Nurse Practitioner at Brant Community Healthcare System (BCHS) to enhance access to supports for individuals with an Opioid Use Disorder, including those who have experienced a recent overdose.  You will respond to individual needs through provision of case management services including assessment, support planning, care coordination, system navigation, provision of harm reduction supplies, and other functions that support stabilization and connection to ongoing care.  You make a difference!   

                    

As an Addictions Case Manager you will:

  • Collaborate effectively with the BCHS Nurse Practitioner and other community partners
  • Respond to referrals in a timely manner, including making initial connection in the hospital or Emergency Department
  • Develop relationship and rapport through person-centred interaction
  • Conduct assessments and collaborate with clients to develop a support and/or safety plan
  • Utilize harm reduction, motivational interviewing and brief-solution focused approaches
  • Facilitate warm connections to ongoing addiction, mental health, and other health and social services
  • Distribute safer substance use and naloxone kits (including coaching on how to use)
  • Collect data to support reporting requirements and continuous quality improvement

 

Annual salary:  $43,953 plus shift premiums and benefits

 

Hours of work:  37.5 hours per week, primarily Monday to Friday days and evenings, with flexibility for weekend shifts as required

 

Start Date:  As soon as possible

TO APPLY, YOU MUST:

  • Have a college diploma in Social Services; University degree in Social Sciences is preferred
  • Have minimum 2 years’ experience working with individuals who use substances, those with mental health issues, and other vulnerable populations
  • Have demonstrated ability to establish client rapport, assess need, develop support plans, mitigate crisis, and navigate health and social service systems to facilitate client connection
  • Demonstrate skill in working collaboratively with service delivery partners
  • Have access to reliable transportation
  • Be able to manage frequent exposure to vicarious trauma and occasional exposure to direct trauma
