The Service Development and Outcomes Specialist is responsible for meeting the vision, mission and values of Epic Opportunities while connecting people living with intellectual disabilities to services offered by the organization.  This includes a person centred approach to developing new services, transition planning, goal setting, facilitating desired outcomes, networking and working collaboratively with families, funders and other colleagues.  The Specialist will facilitate the implementation of Personal Outcome Measures ensuring that individuals are consistently supported to pursue and achieve identified goals.  They will track person centred planning across the organization ensuring that planning frequency meets or exceeds provincial standards. 

This is a part-time (40 hours biweekly) term position until March 31, 2022 with the possibility of extension.

SPECIFIC RESPONSIBILITIES:

Service Development: Promote the work of Epic Opportunities by informing persons with disabilities, their family members, collateral agencies and our funder about our unique expertise and values-based approach to services. Collaborate with referring bodies by responding to their inquiries and needs. Receive and process referrals for service with a focus on best possible outcomes for all people involved. Lead the intake process for day, residential and home share services. Facilitate and oversee all planning related to the transition process including the final transition into the identified services.

Proposal Development: Upon receiving confirmation that services are required and plans have been developed, collect and share all information necessary to the proposal development process. Communicate with the funder as required to advocate for funding approval. Communicate funding progress with all stakeholders. Prepare staffing teams for transition to services.

Advocacy: Promote the right to choice, full inclusion and community participation as well as empower individuals to self-advocate throughout the transition process. Lead the development of creative strategies and partnerships that will support individuals to gain valued social roles, enhance relationships and strengthen natural support networks.

Service Outcomes and Evaluation: Facilitate Personal Outcome Measures work within Epic Opportunities.  Develop tools and systems to formally evaluate services using a quality framework and best practice model.  Engage individuals, their family members and natural support networks, support staff and CSWs regarding desired outcomes. Identify needed resources to improve outcomes.  Ensure that services reflect necessary supports to achieve desired outcomes. Participate in formal service evaluations and promptly bring concerns forward when gaps or barriers are identified.  Engage the support team in solutions. Always involve the person served in discussions of desired or needed changes.  In all matters, ensure that legislative and legal requirements are met.

Human Resource Management: Recruit, screen, secure and provide orientation to new home share providers. Throughout the process, lead them to the accomplishment of our vision, mission and values. Ensure that training and development requirements are met.  Communicate policies and procedures to home share providers engaging in performance management when actions fall short.  Once the home is fully established ensure that the services are seamlessly transitioned to oversight by an Epic Opportunities Coordinator.  Model a standard of excellence in day to day work and provide leadership, direction and guidance to all employees and home share providers.

Administration: Ensure that all required record keeping and financial documentation is completed and remitted in a timely manner, reporting discrepancies according to policy. Safeguard the secure storage of records and documentation in accordance with agency policies and procedures as well as all legislated requirements. Make certain that all services are in compliance with Residential Care Licensing requirements.  Recognize and report all concerns of abuse or neglect.

Financial Responsibilities: Work within the parameters of the budget and other funds connected to service development and outcomes. Approve invoices for payment. Where enhanced financial resources are required secure grants and funding to meet service development and outcome objectives.

QUALIFICATIONS & REQUIREMENTS:

Education & Experience

  • Degree in Social Services or Disability and Community Support Diploma.
  • 3-5 years’ leadership experience in human services.
  • Other combinations of education and experience will be considered.

  • Exhibit excellent verbal and written communication skills, combined with the ability to be compassionate and understanding when responding to service needs and navigating the service system.
  • Be resourceful, creative and imaginative while problem solving service delivery needs and challenges.
  • Network with external professionals and draw expertise from these relationships to inform best practices and meet a quality framework.
  • Continuously engage in organizational improvement to enhance quality of life for all people served.
  • Lead and develop others to assist vulnerable people in achieving personal outcomes.
  • Demonstrate a commitment to community living and full inclusion.
  • A valid Manitoba Driver’s License and use of a personal vehicle is required.
