More than 350 caring professionals work from 20 locations across the city to deliver a vast range of programs and services that support at-risk youth and their families. YSB is a multi-service agency that provides services in French and English in employment, health and housing, mental health, youth engagement and youth justice. Bilingual Transitional Aged Youth (TAY) Case Coordinator

Job Summary:

Under the general direction of the Director of Mental Health Services and the immediate supervision of a Coordinator of YFCS, the TAY Case Coordinator responsible for the provision of assessment and short term counselling services, facilitating client access to adult services, monitoring outcomes and actively coordinating mental health services for youth 16 to 24 years. This position is intended to support youth short term while developing and facilitating intentional pathways for youth who are in need of adult mental health and addiction services and experience challenges in doing so.

 

Principal Duties:

  • Awareness of developments/changes in the community pertaining to the service system and client needs
  • Gather information and review client needs
  • Complete approved screening/ assessment tools with clients
  • Utilize recognized therapeutic interventions to deliver short term counselling/psychotherapeutic support to clients
  • In partnership with youth and service partners, match the treatment options to the clinical needs of the youth
  • Prepare and facilitate case planning meetings
  • Determine/Invite appropriate members for case planning meetings
  • Develop client service plans
  • Promote collaboration between various service providers in order to develop flexible and creative service plans
  • Advocate for access to services on client’s behalf
  • Monitor service plan implementation and outcome
  • Analyze program evaluation feedback and outcome data to develop reports
  • Ensures both electronic and physical client files are up to date, maintained and handled within established practices
  • Follow safe work practices as established by YSB and the Program

 

Job Specifications / Qualifications: (The qualifications are appended to the job description and are not intended to be part of the job description)

  • Bachelors degree in Social Work/Psychology or relevant field coupled with membership in a registered college authorized to practice Psychotherapy
  • Minimum two years related experience in providing counselling services to youth and case management
  • Knowledge of local youth and adult mental health and addictions services
  • Knowledge of local community and social service system
  • Demonstrates decision making and problem solving skills
  • Demonstrates ability to develop a joint case formulation, set goals and assess readiness for change
  • Proven organizational and administrative skills
  • Demonstrates strong interpersonal skills and collaborative abilities
  • Experience in group facilitation
  • Oral and written fluency in English is essential.
  • Oral and written fluency in French is essential
  • Availability and ability to work evenings, flexible hours
  • Broad cross-sectoral experience would be an asset
  • Demonstrated computer skills
  • Valid driver’s license and access to a vehicle
