NYDP Summary:

North Yorkers for Disabled Persons (NYDP) provides permanent supportive housing and around-the-clock support services to individuals with communication and physical disabilities who direct their own services.  NYDP enables an independent lifestyle in a congregate setting. Communication Facilitation and Attendant services are provided 24/7/365. NYDP also houses a Communication Enrichment Program that takes place twice weekly which allows members of the community who use Augmentative and Alternative Communication a place to learn and network together. 

  

Job Description/Responsibilities:

Our Information, Resource and Referral (IRR) Support Program is coordinated by the Outreach Communication Facilitator (OCF) who will provide communication facilitation on an outreach basis to consumers who do not receive services from North Yorkers for Disabled Persons. The position will also provide backup facilitation support to the Manager of Client Services for our residents, as needed. The role of the OCF is to pave the road for effective interaction in the community. The OCF focuses on supporting community members who require facilitation due to a communication disability however, other persons who require general information and referral are able to access the support of the OCF.

 

The OCF will:

  • Provide information on various queries concerning a multitude of topics (housing options, how to apply for programs, etc.)
  • Assist in finding specialized programs and resources
  • Assist with filling out and submitting applications (ASAC, DF, etc.)
  • Provide facilitation at various appointments including medical, employment, Service Ontario/Canada, ODSP etc.
  • Provide access to COVID-19 resources such as testing and vaccination
  • Facilitate large group events that include persons with a communication disability to maintain inclusiveness
  • Work with individuals and families on strategies for self-advocacy and self-direction
  • Partake in the process of transitioning individuals from LTC into the community
  • Conduct assessments for the Attendant Services Application Centre (ASAC)
  • Provide support to the Communication Enrichment Program housed at NYDP
  • Assist with other scenarios as they arise

Qualifications/Skills

  • Desire to work in the field of community and social services
  • Outstanding communication skills
  • Knowledge of and a willingness to learn about the independent living philosophy, augmentative and alternative communication, supportive housing and attendant services.
  • Flexible and reliable team player
  • Display a respectful and positive attitude
  • Ability to identify and immediately respond to urgent situations such as a medical emergency.
  • A commitment to eliminating barriers for people with a communication disability.
  • Access to a personal vehicle is an asset
  • CPR/First Aid

Education: Holds post-secondary education in a health or social care discipline or related field.

Job Type: Full Time. Hours may vary due to client needs. Occasional evening and weekend availability required
