About us:

Founded in 1960, the Youth Services Bureau of Ottawa (YSB) is one of the largest and most comprehensive non-profit agencies and a registered charity. It provides mental health services, employment programs, emergency shelter, community, health and housing services, and justice programs to youth in the Ottawa area, serving between 2,500 and 3,000 at‑risk individuals aged 12+ and their families every month. YSB employs 350 caring professionals in a unionized environment in over 26 locations across Ottawa.

The TAY Case Coordinator is responsible for the provision of assessment and short term counselling services, facilitating client access to adult services, monitoring outcomes and actively coordinating mental health services for youth 16 to 24 years. This position is intended to support youth short term while developing and facilitating intentional pathways for youth who are in need of adult mental health and addiction services and experience challenges in doing so.

 

What will you do:

  • Be aware of developments/changes in the community pertaining to the service system and client needs
  • Gather information and review client needs
  • Complete approved screening/ assessment tools with clients
  • Utilize recognized therapeutic interventions to deliver short term counselling/psychotherapeutic support to clients
  • In partnership with youth and service partners, match the treatment options to the clinical needs of the youth
  • Prepare and facilitate case planning meetings
  • Determine/Invite appropriate members for case planning meetings
  • Develop client service plans
  • Promote collaboration between various service providers in order to develop flexible and creative service plans
  • Advocate for access to services on client’s behalf
  • Monitor service plan implementation and outcome
  • Analyze program evaluation feedback and outcome data to develop reports
  • Ensure both electronic and physical client files are up to date, maintained and handled within established practices
  • Follow safe work practices as established by YSB and the Program

  • Hold a bachelors degree in Social Work/Psychology or relevant field coupled with membership in a registered college authorized to practice Psychotherapy
  • Possess minimum of two years related experience in providing counselling services to youth and case management
  • Have sound knowledge of local youth and adult mental health and addictions services
  • Are familiar with local community and social service system
  • Demonstrate decision making and problem solving skills
  • Demonstrate ability to develop a joint case formulation, set goals and assess readiness for change
  • Possess proven organizational and administrative skills
  • Demonstrate strong interpersonal skills and collaborative abilities
  • Have experience in group facilitation
  • Are fluent in English.
  • Are available and able to work evenings, flexible hours

  • Have broad cross-sectoral experience would be an asset
  • Possess demonstrated computer skills
  • Hold a valid driver’s license and have access to a vehicle
  • Are fluent in French. Bilingualism will be considered an asset, but it’s not mandatory.
