PLEA Community Services is currently seeking a Regular Full-Time Community Inclusion Worker for our FolkStone-NWT team. The proposed start date for this position is October 18, 2021. This position requires BCGEU membership.
The residents who access our FolkStone program are adults who have complex personal and social needs that require a heightened level of support, health care, medical supervision, and specialized services. The incumbent is responsible for assisting residents to participate in educational, vocational, social, recreational, and cultural activities that are available in their communities - and to achieve their individual goals, as identified in their service and nursing plans.

Key Duties and Responsibilities:
- Provides residents with opportunities to access available resources in the community by supporting their participation in social, recreational, cultural, educational, and vocational activities.
- Assists residents to function more independently in the community by encouraging them to develop the necessary personal, social, and life skills, and to achieve their individual goals, as identified in their service and nursing plans.
- Drives and/or accompanies residents to appointments, meetings, and other activities as directed.
- Maintains on-going positive relationships with Family Caregivers and participates as part of the team responsible for enhancing the quality of life for residents and addressing their individual strengths, needs, abilities, and preferences consistently.
- Develops collaborative relationships with residents' families/caregivers, significant others and informal support systems to maintain and strengthen these connections.
- Engages the residents in acceptable recreational and social activities in the community that will encourage new interests, strengthen their resilience and develop their interpersonal communication skills. Teaches life skills and community survival skills, provides emotional support, and models acceptable behavior.
- Adheres to all policies and procedures established by PLEA, and meets the performance expectations of the position in a manner that demonstrates compassion and respect for all persons served. Exercises the responsibilities of the position in a manner consistent with the philosophical approach and mandate of the program.
- Performs other related duties as required.
Education, Training, and Experience:
- At least a two-year diploma from a post-secondary institution in the health or social services field.
- Recent volunteer or work experience working with individuals who require specialized supports and guidance to participate in the life of their community.
- Demonstrated proficiency in Microsoft Windows, Microsoft Office, and familiarity with database programs.
Job Skills and Abilities:
- Good interpersonal communication skills, including a demonstrated ability to write accurate notes.
- Demonstrated ability to work with others; enjoys the company of adults and feels comfortable working with those living with developmental disabilities, an acquired brain injury, FASD, dementia, and other mental health issues.
- Demonstrated ability to develop supportive relationships with residents and their families/caregivers, and to be sensitive and understanding of their social realities including issues of race, religion, culture, gender identity, sexual orientation, poverty, and lifestyle.
- Demonstrated ability to encourage and support adults to participate in community activities, use good judgement, and to follow all instruction and protocols.
- Ability to work collaboratively on a team with other program staff and with others in the community including professionals, collaterals, and families/caregivers, informal supports and community members. Demonstrated ability to work independently and be accountable for results.
- Satisfactory Criminal Record Clearance.
- Must possess a valid Occupational First Aid Level 1 Certificate or be willing to obtain one prior to hire.
- Use of Personal Vehicle. This position requires travel in the Lower Mainland, the Northwest Territories and the Fraser Valley.
- A valid B.C. Driver's License (Class 5 or Class 7), and suitable driver's abstract.
- The usual hours of work in this position are Monday to Friday, 9:00AM to 5:00PM, based on 37.5 working hours per week.
