Reporting to the Chief Executive Officer, the Nurse Practitioner will support the delivery of clinical care to the 280 residents of our Long-Term Care facilities (Grace Manor and Faith Manor). The Nurse Practitioner will be tasked with providing direct resident care as well as, education and leadership to the registered nursing staff, and will collaborate in research, education and evidence-based practice initiatives within the multi-disciplinary team.

Specific responsibilities include:


  • Provide comprehensive primary care to our residents as part of the interdisciplinary health care team
  • Manage the care of residents by providing pharmacological, complementary and/or counselling interventions, and perform procedures within the NP scope of practice
  • Provide exceptional person-focused health education
  • Conduct person-centred health assessment of residents on admission, annually and as needed
  • Order and/or perform appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up
  • Identify, develop and implement practice innovations, in collaboration with the Medical Director(s)
  • Provide formal and informal teaching and coaching in the management of clinical care to the interdisciplinary team members, serving as a resource person, educator and role model
  • Oversee the RNAO Best Practice Spotlight Organization (BPSO) work at both Long-Term Care facilities in collaboration with program leads
  • Champion the Quality Improvement Program for the home as it relates to Skin and Wound Care, Falls Management, Continence Care, Behavioural Supports, etc.
  • Other duties as required



  • Current registration with the College of Nurses of Ontario as a registered nurse in the extended class, and entitled to practice
  • A minimum of three (3) years experience working in a long-term care setting, or an equivalent amount of education and experience.
  • Post-graduate education and experience in Gerontology is an asset.
  • Specialty certification in geriatric nursing is preferred.
  • Experience in advanced care planning, palliative care and end of life care delivery is preferred
  • Current member of RNAO or NPAO with liability coverage.
  • Proven experience in chronic disease management and dementia care is an asset.
  • Strong interpersonal and communication skills.
  • Proven charting and documentation skills, with exceptional attention to detail
  • Prior exposure to PointClickCare is an asset.
  • Ability to speak Dutch is an asset
  • Must be able to sit, stand and walk for long periods of time
  • Knowledge of legislative requirements for long term care and accreditation guidelines.
  • A comfort level in maintaining the core values and traditions of Holland Christian Homes as a Christian Homes
  • Satisfactory Vulnerable Sector Screening.
  • Evidence of a negative TB including flu shot and full COVID-19 vaccination.
