OVERVIEW: The Reunification Homes Program is currently recruiting for a Transitional Family Mentor. This role specializes in providing family enhancement and prevention support to families and their children. The Transitional Family Mentor will provide services to families prior to move in and after move-in to Reunification homes. The successful applicant will connect families to community-based resources and supports, support families to ensure a structured and nurturing home environment, support the development of positive parenting, and will work collaboratively with families and agencies to set goals and coordinate service delivery to meet identified needs.

 

 

PRE-UNIFICATION DUTIES:

  • Connect with families on the Reunification Homes waitlist to assist with developing a network of support
  • Collaborate with SAI Wellness Team to ensure that families are supported in meeting their physical, emotional, mental, and spiritual needs
  • Be a healthy role model and source of guidance for the development of child and family daily life skills and healthy parenting skills
  • Research and access additional family resources in the community that would benefit families and their children
  • Work collaboratively with SECFS on identified family needs
  • Maintain clear and concise records and provide reporting as necessary

 

POST-UNIFICATION DUTIES:

  • Support and mentor the family in providing a nurturing and structured family environment with positive attachment caregiving
  • Collaborate with care team to identify needs and prepare for successful transition for families from the Reunification Home to living independently (house set up, housing, EIA, school enrollment)
  • Ensuring post Reunification support team is in place and monitoring on-going goal setting in the home
  • Facilitate weekly in-house checks ins
  • Facilitation parent support group meetings

 

QUALIFICATIONS:

  • Previous experience as a Support Worker or experience working with families from a strength-based, and trauma-informed perspective
  • Post-secondary education in the Human Services field and/or experience working in the helping field providing direct support to parents and children from a trauma informed perspective
  • Knowledge in addictions, homelessness, attachment, intergenerational trauma, domestic violence and strong understanding of child development and impact of trauma
  • Knowledge of holistic practice incorporating the seven sacred teachings and using the quadrants of the medicine wheel
  • NVCI, CPR First Aid, and ASIST training considered an asset
  • Strong communication and organizational skills and ability to exercise sound judgement and good decision making
  • Ability to work both independently and collaboratively with community resources, collateral agencies, and schools to set goals and coordinate service delivery to meet identified needs
  • Maintains a high level of discretion and confidentiality
  • Driver’s license and access to a vehicle
  • Must provide clear Criminal Record check, Adult and Child Abuse checks, Prior Contact Check, and clear drivers abstract
  • Has knowledge and understanding of Indigenous culture and ways of life

 

WORK SCHEDULE:

Hours of work will generally be 8 hours per day and 40 hours per week Monday to Friday, 8:30 am to 4:30pm. Occasional evening hours may be required. Occasional overnight travel to our communities may be required.
