Support and Care Navigator

Part-Time – 20 hrs/week- w/rotating schedule


The Support and Care Navigator shall work within the mandate of

Women’s Health in Women’s Hands Community Health Centre


  1. Act as part of the circle of care performing intake interviews, supportive counselling, advocacy, systems navigation and crisis intervention functions to individuals, groups and the community
  2. Conducts client assessments such as GAD7’s/PHQ9’s, screenings and coordinated care plans
  3. Provides a comprehensive introduction to all eligible services and a range of instrumental supports (forms, interpretation, and internal referrals)
  4. Assist in client crisis and suicide intervention
  5. Strong knowledge of the social determinants of health and issues that affect racialized communities
  6. Provide client advocacy services and broker/mobilize available social, financial, and institutional resources e.g. external referrals and information regarding community services, shelters, government services, partner agencies; assists clients with access to items such as food clothing and needle exchange
  7. Engage community groups and networks in support of community initiatives
  8. Registers clients and collects socio-economic demographic and other relevant information
  9. Ensures that required documentation is completed and available in the client’s health record in a timely manner that meets professional standards of practice.
  10. Coordinate and align health promotion initiatives with delivery of community health education programs organization wide, including health education in primary and allied care.
  11. Initiate and coordinate participate in program and chart review and case conferencing with a continuous quality improvement (QI) focus.
  12. Collaborates and delivers health promotion, education, prevention, and workshops
  13. Participates in development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, groups and the community.
  14. Other duties as assigned.




  1. Bachelor of Social Work (B.S.W.) or equivalent degree and proven experience
  2. Three to five years relevant experience in a community setting.
  3. Ability to work both independently and collaboratively within an interdisciplinary team.
  4. Proficiency in current assessment, counselling, case management and crisis intervention techniques
  5. Excellent communication skills, verbal and written and the ability to prioritize and multi-task.
  6. Knowledge of relevant providers, their policies, and support networks
  7. Ability to establish good working relationships with partner agencies.
  8. Experience in program development, implementation, monitoring and evaluation
  9. Strong working knowledge of women’s health issues.
  10. Excellent assessment skills and strong clinical judgment
  11. Proficiency in the use of computers, Microsoft Office (Excel, Work, PowerPoint, Outlook), and various other software applications
  12. In addition to English, knowledge of a language spoken within one of the four priority communities, an asset
