Mental Health Trauma Case Manager 

One .5fte (17.5 hrs/wk) permanent position

 

In this role, you will provide comprehensive community based case management response and advocacy services to individuals age sixteen and over with serious mental illness within York Region. This position is designed to provide designated case management support to the clients of Cedar Center, Adult Program formerly known as York Region Abuse Program. The Mental Health Trauma Case Manager will facilitate recovery and assist service users to improve their quality of life through a collaborative approach to service provision through community visits.

 

A hybrid remote work arrangement is available for this position.

 

Requirements
  • Bachelor’s degree in Social Work or related discipline (i.e. Psychology, etc.)
  • Minimum 3 years related experience required
  • Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills are assets
  • Proficiency in French is considered an asset.  
  • Trauma informed case management knowledge is an asset.
  • Counselling, crisis prevention and intervention experience
  • Experience working with individuals having serious mental illness is essential
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid G driver's license, appropriate insurance coverage for business use, and reliable vehicle are required for this position
