Project: Integrated Community Services for Seniors (ICSS)

About the Position:

The Project Lead is responsible in providing leadership for planning and implementation of the daily operation of the community hub, including but not limited to, recruitment and staffing, project planning and data analytics. Liaises with community services to build capacity for the program. Key partnerships are with Vancouver Coastal Health (VCH). The Project Lead is also responsible for event planning and familiarity of senior services in the Richmond area.

Reports to: Chief Executive Officer

Key Duties & Responsibilities:

  • Leads the Project team to commission the community hub
  • Participates in meetings with VCH care providers to discuss the need of Chinese clients and their families to access hospital and community-based resources and document the information needs necessary for culturally specific and sensitive care across the continuum of care
  • Manages and resolves any staffing and client issues by seeking satisfactory resolution on a timely fashion
  • Adopts best practice approach to customer service by developing rapport with clients and community stakeholders
  • Acts as an ambassador in the community to promote the integrated service hub for seniors
  • Develops policy and procedure to guide best practice and provides training to staff and community stakeholders
  • Maintains documentation and a database of client interactions
  • Works collaboratively with the program Researcher to determine data collection and analysis for interim and final reporting as well as policy paper
  • Develops a monthly roster of relevant webinars for seniors and families
  • Coordinates with speakers for webinars and provides evaluation for its effectiveness
  • Strives to improves the efficiency and effectiveness of the community service hub and overall project management

Qualifications & Experience:

  • Bachelor’s Degree in health care or equivalence, Social Work, Nursing or related discipline
  • Minimum of three (3) years’ recent and related experience working with Chinese communities in British Columbia
  • Extensive knowledge of community-based Chinese organizations in Richmond, BC
  • Extensive knowledge of health issues impacting Chinese communities
  • Broad knowledge of Chinese and non-Chinese services and resources currently available within Vancouver Coastal Health’s (VCH’s) Health Services Delivery Areas (HSDA’s) for Chinese clients in need of health and social supports

Job Skills & Abilities:

  • Ability to communicate effectively both in English and Chinese verbally and in writing
  • Ability to deal with cultural issues sensitively
  • Leadership ability to initiate and maintain collaborative working relationships
  • Intermediate skills with operating Word processing, Excel spreadsheet, and database programs
  • Excellent skills in organization and project management
  • Excellent skills in customer service
  • Leadership ability to connect with seniors’ services and other relevant health/social services in the community
  • Ability to build and manage connections with seniors’ services
  • Ability to host webinars/events (in-person and virtual)
  • Excellent problem-solving and conflict resolution skills
  • Ability to develop rapport with clients and community stakeholders
  • Excellent critical thinking skills to plan and implement risk mitigation strategy

Other Requirements:

  • Criminal Record Check Required
  • Ability to work on evenings/weekends when required
  • Travel within Richmond area is required
