Healthcare Navigator : Addictions
Full-time (35 hours/week), Contract position
May 2022 – March 2023
Job Summary
The Healthcare Navigator will facilitate access to services and case management for individuals living in Northwest Toronto who need addictions supports. The Navigator will serve as a critical link between mental health and addictions services, primary and acute care, and social services to facilitate smooth transitions across the healthcare system for individuals and their caregivers. The Navigator will work closely with the Wellness Team to ensure timely access to services and work with providers to address social determinants of health barriers. The Navigator will also assist with data collection and analysis, and in the development of quality and process improvement initiatives.
Key Responsibilities
Facilitate timely referral pathways for addictions services and programs for individuals
Training and distribution of naloxone, distribution of harm reduction equipment, low-barrier witnessing to allow for early intervention in case overdose occurs, and harm reduction education and training
Provide low threshold access to services and case management, including provision of services at times when people use drugs, including weekends and nights when other services are closed
Identification of people within service sites (e.g., shelters/respites/hotels) for further training to act as onsite resources
Act as support/liaison/navigator for individuals along their care journey
Ensure clear and timely communication between care team providers
Assist and empower clients to engage in self-management of condition(s) by establishing relationships, follow up, and providing resources as needed
Complete timely documentation utilizing identified electronic record software
Support quality improvement initiatives through identification, management, and compilation of data metrics to measure impact
Ensure the use of evidence based, health literate educational materials
Participate in case conferences and team planning meetings as required
Handle other related duties as assigned.
Qualifications
Social Work degree from a recognized university
Minimum of three years’ work experience in social services or community health setting
Experience working with, and extensive knowledge of local social services, health care organizations and community resources relevant to the client population
Experience working in culturally, socially and economically diverse communities
Excellent interpersonal, oral and written communication skills
Proven understanding and knowledge of working with persons with mental health and/or addictions issues as well as an understanding of the intersectionality of systemic barriers and social determinants of health
Demonstrated ability to exercise sound judgement and decision-making skills
Demonstrated initiative and experience working independently with minimal supervision
Fluency in English and language(s) common to Northwest Toronto (Spanish, Vietnamese, Italian) considered an asset
Proficiency with Microsoft Office programs, EMR and ability to learn and master other computer programs as needed
Flexibility to work evenings, weekends and at various locations
