The Opportunity

 

Foundry is removing barriers and increasing access to health and wellness services for young people aged 12–24 and their caregivers through a network of youth-friendly centers across British Columbia and online. By offering integrated mental health care, substance use services, primary and sexual health care, youth and family peer support and social services, Foundry makes it easier for young people to find support in their communities. Online resources and first-of-its-kind virtual care further broaden Foundry's reach.

 

The Program Coordinator is responsible for the oversight and coordination of Foundry Langley daily activities and services. They provide coordination ,supervision and promote interdisciplinary practice and collaboration. The Program Coordinator ensures quality care that is informed by youth needs and preferences and promotes continuous quality improvement through all systems and processes. They collaborate deeply with related health and social support services and other relevant community resources. They also participate actively in the Foundry provincial network and related working groups and events.

SALARY RANGE:                         Grid 14 Step 2, $29.74

Key Duties and Responsibilities:

 

  1. Plays a key role in ensuring that the Centres offer a safe, welcoming, inclusive and youth-friendly environment;
  2. Coordinates, the day-to-day centre operations by ensuring the necessary facilities and equipment are in place, program guidelines and policies are adhered to and program standards are met in consultation with the Clinical Coordinator; and Clinical Manager;
  3. Coordinates staff scheduling and programming, in collaboration with the Clinical Coordinator;
  4. Provides day-to-day supervision and support to all members of the integrated service delivery team working closely with coordinators of partner agencies and clinical supervisors;
  5. Supervises program staff by performing duties such as assigning work, providing feedback on performance, and conducting performance evaluations.
  6. Ensure the programs offer high quality, inclusive, participant-centered and culturally competent services utilizing ethical practices that are responsive to the needs of presenting participants;
  7. Provides direct participant care or process guidance when indicated;
  8. Ensures effective case planning and management and accountability to service partners;
  9. Nurtures a positive team environment of continuous learning, effective communication and service excellence;
  10. Supports program workforce development including the recruitment, selection, orientation, supervision, scheduling and professional development; Works closely with the Foundry Central Office team to ensure effective communication, knowledge exchange, data collection and evaluation practices;
  11. Ensures best practices, such as Foundry’s Integrated Stepped Care Model, team-based care and peer support, are at the core of service delivery;
  12. Supports the Clinical Manager in the development of the budget and informs senior management of budgetary needs;
  13. Recognizes and manages risk to staff and youth, and ensures actions are taken to prevent and minimize harm;
  14. Responds to and provides support to team members and colleagues during critical incidents and high-risk situations; available for assistance in crisis interventions, debriefing, and consultation;
  15. Performs administrative functions, including writing reports, producing documents and presentations, ensuring adherence to policies;
  16. Oversees use of shared participant documentation system, Toolbox, and Electronic Medical Record and ensures accountability and privacy;
  17. Maintains an in-depth knowledge of available resources in the community;
  18. Liaises and maintains positive working relationships with community funders (MCFD, Foundry BC ) and other community stakeholders and resources;
  19. Represents Foundry in the community and delivers public presentations;
  20. Plan and or participate in team meetings, community committees, organizational meetings/initiatives, task groups, continuing education and/or community initiatives by methods such as sharing up-to-date information on programs, services and referral processes; acting as an advocate for the goals and objectives of the team; facilitating the development of clear, efficient service and referral pathways; and promoting optimal support for participants and their informal support network to improve access, integration and coordination of services;
  21. Formulates and recommends the development of new program areas related to peer support and social services;
  22. Supports the coordination and facilitation of engagement initiatives, including the Youth Advisory Committee and Family Advisory Committee, and brings forward recommendations;
  23. Maintains a consistent communication strategy to ensure continuous community awareness;
  24. Formulates program policies and procedures and ensures compliancy with agency policies and procedures and standards of accreditation in consultation with the Clinical Coordinator and Program Manager;
  25. Reports any breach of confidentiality, health and safety concerns pertaining to participants, participants, staff and volunteers;
  26. Coordinator specialized services/programs with social support and peer support, as required, including supporting a gender-affirming care clinic; and
  27. Performs other duties as required/assigned.

Qualification

1.  Education and Knowledge
  • Bachelor’s degree in human/social services or related field.
2.  Training and Experience
  • Three (3) years direct service delivery experience in the community social services sector, including one (1) year supervisory or administrative experience . Or an equivalent combination of education, training and experience.
  • Experience with multi-disciplinary health care teams and primary care an asset;
  • Understanding of and passion for supporting youth and their families;
  • Genuine commitment to service integration and improving service experiences for youth and their families;
  • Experience with developing policies and procedures, designing and delivering programs a significant asset; and
  1. Skills and Abilities
  • Strong communication, decision-making and organizational skills;
  • Solid foundation in human resources and occupational health and safety practices and related legislation;
  • Knowledge of best practices related to community health and social services, including but not limited to harm reduction, stepped care, cultural safety and youth and family centered care;
  • Staff mentoring and development skills, ability to coordinator a positive working environment;
  • Relationship and team building skills with a wide variety of stakeholders;
  • Excellent self-management skills in high pressure situations;
  • Ability to analyze and resolve problems with minimal direction;
  • Ability to thrive in a dynamic, changing environment;
  • Physical ability to complete required tasks;
  • Demonstrated knowledge and application of trauma-informed and culturally safe principles of care;
  • Demonstrated knowledge of relevant Acts, pertinent regulations, policy, procedures and other statutory and certification requirements (such as the Mental Health Act; Adult Guardianship Act; Health Care (Consent) and Care Facility (Admission) Act; Child, Family and Community Service Act; and Freedom of Information and Protection of Privacy Act);
  • Effective listening and consensus building skills that promote communication and coordinator to a cooperative approach to problem solving and decision making within interdisciplinary settings;
  • Ability to work effectively with others in a multicultural environment, independently and as part of an inter-professional team, including understanding the frameworks of other disciplines;
  • Ability to manage crisis situations and provide supervision to staff in the management of participant crises;
  • Ability to maintain professional integrity, boundaries, and confidentiality;
  • Ability to make effective referrals in a collaborative manner, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community partners; and
  • Knowledge of systemic issues and risk factors facing minority groups including LGBTQ2S+ and Indigenous youth and young adults;
  • Knowledge and respect of the diversity of the BC population;
  • Demonstrated organizational and time management skills, accountability, reliability and punctuality; Strong planning, organizational and administrative skills with the ability to prioritize tasks and manage numerous tasks simultaneously;
  • Willingness to work flexible hours including evenings;
  • Proficiency in computer applications: experience with Microsoft Office, Excel, PowerPoint, Outlook, CAMS and EMRs.
  • Physical ability to carry out the duties of the position.

4. Other Information

  • Must possess a valid BC class 5 Driver’s license, have a reliable vehicle, business insurance and drivers abstract review (mileage paid for work use);

  • Evidence of training in Telehealth and/or virtual services;
