POSITION OVERVIEW

A leader in community palliative care service, Hospice of Waterloo Region operates a 10 bed hospice residence in North Waterloo. This new facility provides much needed hospice service to individuals across the Region of Waterloo. We are hiring a Clinical Resource Nurse with a passion for providing hospice palliative care and a commitment to providing holistic care to clients in our residence.  As an experienced RN, you have strong leadership skills, coupled with an ability to respond to the needs of clients at end of life with compassion and expertise when faced with varied clinical conditions and diagnoses. You are accustomed to helping families navigate the death of a loved one with empathy and an understanding that caring for the family is central to excellent palliation.  You have strong communication skills, are used to working on an interdisciplinary team and comfortable working within a team that includes a strong core of volunteers.  

 

POSITION SPECIFIC RESPONSIBILITIES

  • Contribute to a holistic supportive environment to meet the diverse needs of clients and families
  • Review and oversee individualized, holistic care plans consistent with the 9 domains of palliative care
  • Work with the Director of Care and intake coordinator to complete suitability assessments and admissions including liaising with external providers prior to admission
  • Coordination with the DOC on staffing needs, staff schedules, and staff work flows
  • Develop and deliver orientation to new staff
  • Work with the DOC to strengthen the clinical skills of the residence staff by providing education to new and existing staff regarding policy changes or best practice treatment methods
  • Establish a schedule of chart audits to ensure high standards of documentation of all patient files
  • Work collaboratively with the RN, RPN and PSWs to provide care for complex patients with stable and unstable symptoms
  • Provide nursing care, as required, including pain management, wound care, etc. in a professional and compassionate manner
  • Administer oral and subcutaneous medications, initiation and titration of patient controlled analgesia pumps
  • Complete and maintain stock of clinical supplies

REQUIRED QUALIFICATIONS AND EXPERIENCE

  • Registered Nurse in good standing with the College of Nurses of Ontario
  • Canadian Nurses’ Association Hospice Palliative Care Nursing Certification (CHPCN(C)) (or a commitment to obtain) preferred
  • Completion of LEAP, CAPCE/Fundamentals of Palliative Care an asset
  • Minimum 5 years working as an RN
  • Minimum 3 year experience in Hospice Palliative Care in community, hospital or hospice environment
  • Previous leadership/management experience an asset

 

SKILLS AND ATTRIBUTES

  • Knowledge and proficiency in current, evidence-based methods and practices of hospice/palliative care and delivery
  • Ability to prioritize competing tasks working autonomously and within a team
  • Experience delegating tasks and working in an interdisciplinary team
  • Proficiency in the use of the computer hardware & software, particularly in Microsoft Word, Excel, Outlook.
  • Familiarity with electronic medical records and documentation
  • Open, flexible and non-judgmental
  • Commitment to lifelong learning
