SCHEDULE: Tuesday - Saturday, 8:30am-4:30pm. Schedule must be flexible to meet organizational needs.

SALARY: $42,638.73- $51,908.39 annually, depending on qualifications and experience

JOB SUMMARY

The Many Ways Home Housing Outreach Worker is a member of the Phoenix team in the Abbotsford Community Hub and participates in the recovery and capacity building of residents and alumni to maximize their social and economic potential and support their goals for successful independent living in the community.  

KEY RESPONSIBILITIES

• Provide case management services to Participants accessing the service 
• Prepare or assist with housing applications and admissions documentation for review. Identifies areas in which participants would benefit from supportive assistance, and funding streams
• Ensure ongoing assessment of client progress regarding levels of support needed.
• Support individuals through crisis including tenancy breakdown.
• Form and maintain effective collaborative relationships of accountability on actions plans with guests, for the purpose of facilitating the Participants’ success. 
• Actively involved in the participants action planning and encourages the participant to take responsibility for the outcomes he or she is striving to achieve.
• Monitor and document Participants’ progress, maintains and updates client case management files through a database system ensuring that monthly reporting deadlines are met. 
• Transport and accompany clients to housing searches, and appointments that fall within the essential need when client is unable to do so themselves.
• Assist in liaising with treatment centres, support recovery programs, addictions and mental health workers, detox, shelters, primary care clinics, corrections, police, hospitals, outpatient clinics and other supports to reach prospective residents to improve continuity of care.

QUALIFICATIONS

• Academic: Minimum requirement of 2 year diploma in social work or an equivalent field of study.
• At least one year work experience in a related field
• English: fluent and able to effectively communicate professionally, orally and written in English. 
• Satisfactory Tuberculosis Screening Certificate: current negative test 
• First Aid Certificate with CPR: current 
• Satisfactory Criminal Record Check Review - through the Ministry of Public Safety and Solicitor General Criminal Records Review Program.
• Driver’s License: Valid BC license with business insurance, accompanied by a driver’s abstract.
